冠心病患者握力测试和运动能力的研究

摘要 目的 探讨冠心病患者握力和运动能力的变化。方法 选取2020年2月至2021年1月于绍兴市人民医院就诊行冠状动脉CT血管成像明确诊断的冠心病患者100例纳入冠心病组,另选取经冠状动脉CT血管成像排除冠心病诊断者100例纳入非冠心病组。比较两组研究对象的握力测试、6分钟步行试验及心肺运动试验结果。结果 两组研究对象的性别、年龄、高血压病史、糖尿病病史、吸烟史、体质量指数、甘油三酯、高密度脂蛋白胆固醇、左心室射血分数、左心室舒张末期内径比较差异均无统计学意义(P>0.05);冠心病组患者的总胆固醇、低密度脂蛋白胆固醇水平均显著高于非冠心病组(P<0.05);规律运动比例、无氧阈时的摄氧量、无氧阈时的代谢当量、峰值代谢当量及峰值摄氧量均显著低于非冠心病组(P<0.05),握力显著小于非冠心病组(P<0.05),6min步行距离显著短于非冠心病组(P<0.05)。结论 与冠心病患者相比,非冠心病者有更好的握力、心肺功能及运动能力,可能与运动习惯、胆固醇水平有一定相关性,握力测试可考虑作为运动能力测试的重要补充或替代。 Objective To explore the changes of grip strength and exercise ability in patients with coronary heart disease. Methods From February 2020 to January 2021, 100 patients with coronary heart disease diagnosed by coronary artery computed tomography angiography in Shaoxing People’s Hospital were selected into coronary heart disease group, and 100 patients with coronary heart disease excluded by coronary artery computed tomography angiography were selected into non-coronary heart disease group. The results of grip strength test, 6 min walk test and cardiopulmonary exercise test were compared between the two groups. Results There were no significant differences in sex, age, hypertension history, diabetes history, smoking history, body mass index, triglyceride, high density lipoprotein-cholesterol, left ventricular ejection fraction and left ventricular end diastolic diameter between the two groups(P>0.05). The levels of total cholesterol and low density lipoprotein-cholesterol in coronary heart disease group were significantly higher than those in non-coronary heart disease(P<0.05), proportion of regular motion, anaerobic threshold oxygen uptake, anaerobic threshold metabolic equivalent, peak oxygen uptake and peak metabolic equivalent were significantly lower than those in non-coronary heart disease group(P<0.05), the grip strength was significantly lower than that in non-coronary heart disease group(P<0.05) and the 6-minute walking distance was significantly shorter than that in non-coronary heart disease group(P<0.05). Conclusion Compared with patients with coronary heart disease, patients without coronary heart disease have better grip strength,cardiopulmonary function and exercise ability, which may be related to exercise habits and cholesterol level. Grip strength test can be considered as an important supplement or substitute for exercise ability test.
