不同分型糖尿病合并DGP的临床特征分析

Clinical characteristics of different types of diabetes mellitus complica ted with DGP

目的:分析1型糖尿病(T1DM)、2型糖尿病(T2DM)合并糖尿病性胃轻瘫(DGP)的临床特征及严重程度。方法:纳入糖尿病合并DGP患者45例参与研究。对糖尿病类型、基线人口统计学特征、糖脂代谢数据等临床特征进行分析,以胃轻瘫基本症状指数(GCSI)评分及胃内容物滞留量对胃轻瘫的严重程度进行判断。结果:45例DGP患者中有T1DM患者11例、T2DM患者34例。与T1DM合并DGP患者相比,T2DM合并DGP患者的年龄更大,糖尿病病程更长,体质指数和低密度脂蛋白胆固醇水平均更高,糖化血红蛋白(GHb)水平更低,其中合并心血管疾病患者的占比更高,差异有统计学意义(P<0.05)。与T2DM合并DGP患者相比,T1DM合并DGP患者中重度胃轻瘫患者的占比更高(45.45%vs 23.53%),且其餐后2 h和4 h的胃内容物排空延迟更为明显。结论:不同分型糖尿病合并DGP的临床特征及严重程度存在差异,可能与相应的病理生理学特征有关。

Objective:To analyze the clinical characteristics and severity of type 1 diabetes mellitus(T1DM)and type 2 diabetes mellitus(T2DM)complicated with diabetic gastroparesis(DGP).Methods:A total of 45 diabetic patients with DGP were enrolled in this study.The type of diabetes,baseline demographic characteristics,glucose and lipid metabolism data and other clinical characteristics were analyzed.The severity of gastroparesis was judged by the basic symptom index(GCSI)score and the amount of gastric contents retention.Results:Among 45 patients with DGP,11 patients had T1DM and 34 patients had T2DM.Compared with T1DM patients with DGP,T2DM patients with DGP were older,had longer duration of diabetes,had higher body mass index(BMI)and low density lipoprotein cholesterol(LDL-C)levels,and had lower glycosylated hemoglobin(GHb)levels.The proportion of patients with cardiovascular diseases was higher,and the difference was statistically significant(P<0.05).Compared with T2DM patients complicated with DGP,the proportion of moderate to severe gastroparesis patients in T1DM patients complicated with DGP was higher(45.45%vs 23.53%),and the delay of gastric contents emptying at 2 and 4 hours after meal was more obvious.Conclusion:There are differences in the clinical characteristics and severity of DGP among different types of diabetes mellitus,which may be related to the corresponding pathophysiological ch aracteristics.
