高血压脑小血管病老年患者血管性认知功能障碍程度与IL-18及CCL-2等细胞因子的相关性分析

Analysis of the correlation of vascular cognitive dysfunction with IL-18 and other cells in elderly patients with hypertensive cerebrovascular disease and CCL-2

目的探讨高血压脑小血管病(CSVD)老年患者血管性认知功能障碍(VCI)程度与白细胞介素(IL)-18及趋化因子配体2(CCL-2)等细胞因子的相关性。方法回顾性分析2019年1月—2020年12月韶关学院医学院附属医院和粤北人民医院收治的老年高血压CSVD患者100例的临床资料,根据有无认知功能障碍分为无血管性认知功能障碍组(WVCI)与非痴呆性组(VCIND)。入选者均进行血液检测,取5 mL空腹静脉血检测IL-18、CCL-2、肿瘤坏死因子α(TNF-α)、组织生长因子β(TGF-β),分析上述指标与与认知功能障碍程度的相关性。结果WVCI组IL-18(7.96±0.72)pg/mL、CCL-2(80.93±8.22)pg/mL、TNF-α(53.67±4.82)pg/mL,均低于VCIND组的(55.49±6.34)pg/mL、(453.54±85.71)pg/mL、(73.98±5.34)pg/mL;TGF-β(336.52±61.04)pg/mL,高于VCIND组的(218.14±42.75)pg/mL,差异有统计学意义(P<0.05)。IL-18与认知功能障碍程度呈正相关(r>0,P<0.05);与CCL-2、TNF-α、TGF-β无相关性(P>0.05)。结论老年高血压CSVD患者IL-18水平越高认知功能损伤越严重,两者呈正相关,IL-18能在一定程度上反映认知障碍程度。

Objective To investigate the correlation of vascular cognitive dysfunction(VCI)in elderly patients with hypertension(CSVD)with cytokines such as interleukin(IL)-18 and chemokine ligand 2(CCL-2).Methods 100 elderly patients with hypertension admitted to the Affiliated Hospital of Shaoguan College of Medicine and the People's Hospital of North Guangdong from January 2019 to December 2020,according to the absence of cognitive dysfunction,they were divided into nonvascular cognitive dysfunction group(WVCI)and non-dementia group(VCIND).All blood tests were performed with 5ml fasting venous blood for IL-18,CCL-2,tumor necrosis factor(TNF),tissue growth factor(TGF),to analyze the correlation with the degree of cognitive dysfunction.Results The WVCI IL-18(7.96±0.72)pg/mL,CCL-2(80.93±8.22)pg/mL,TNF-α(53.67±4.82)pg/mL were all below VCIND(55.49±6.34)pg/mL,(453.54±85.71)pg/mL,(73.98±5.34)pg/mL,TGF-β(336.52±61.04)pg/mL is higher than the VCIND Group(218.14±42.75)pg/mL,differdifference are significant(P<0.05);IL-18 is positively associated with the degree of cognitive dysfunction(r>0,P<0.05);No correlation with CCL-2,TNF-α,TGF-β(P>0.05).Conclusion The higher the IL-18 level in elderly hypertensive CSVD,the positive correlation.IL-18 can reflect the degree of cognitive impairment to some extent.
