兰索拉唑为主三联疗法治疗胃溃疡患者效果及对血清胃泌素的影响

Effect of Lansoprazole based triple therapy on patients with gastric ulcer and its effect on serum gastrin

目的探究兰索拉唑为主三联疗法治疗胃溃疡患者的效果及对血清胃泌素的影响。方法采用信封法将新干县人民医院2018年2月—2021年2月收治的60例胃溃疡患者分为对比组和研究组,研究组30例,采用兰索拉唑联合阿莫西林、克拉霉素治疗;对比组30例,采用奥美拉唑联合阿莫西林、克拉霉素治疗。对比2组患者临床治疗效果、溃疡面积、症状改善时间、血清胃泌素、相关因子表达[血管活性肠肽(VIP)、表皮生长因子(EGF)、血管内皮生长因子(VEGF)]以及不良反应发生率。结果治疗前2组患者溃疡面积、临床症状评分、血清胃泌素水平、VIP、EGF、VEGF水平差异无统计学意义(P>0.05);治疗后研究组治疗总有效率较对比组更高,且溃疡面积更小,炎症消退时间、胃痛消退时间、住院时间更短,血清胃泌素水平更低,VIP、EGF、VEGF水平更高,差异均有统计学意义(P<0.05);2组不良反应总发生率差异无统计学意义(P>0.05)。结论兰索拉唑为主三联疗法可有效提高胃溃疡患者的临床治疗效果,促进症状康复,降低胃泌素水平,促进胃黏膜修复,且安全性高,值得临床推广。

Objective To explore the effect of lansoprazole based triple therapy on patients with gastric ulcer and its effect on serum gastrin.Methods 60 patients treated in our hospital from February 2018 to February 2021 were divided into control group and study group by envelope method,of which 30 patients in the study group were treated with lansoprazole combined with amoxicillin and clarithromycin,and 30 patients in the control group were treated with omeprazole combined with amoxicillin and clarithromycin.The clinical treatment effect,ulcer area,symptom improvement time The expression of serum gastrin and related factors[vasoactive intestinal peptide(VIP),epidermal growth factor(EGF),vascular endothelial growth factor(VEGF)]and the incidence of adverse reactions.Results Before treatment,there was no significant difference in ulcer area,clinical symptom score,serum gastrin level,VIP,EGF and VEGF level between the two groups(P>0.05).After treatment,the total effective rate of the study group was higher than that of the control group,and the ulcer area was smaller,the inflammation subsided time,stomach pain subsided time and hospital stay were shorter,the serum gastrin level was lower,VIP,EGF The level of VEGF was higher,and the difference was statistically significant(P<0.05);There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Lansoprazole based triple therapy can effectively improve the clinical treatment effect of patients with gastric ulcer,promote the recovery of symptoms,reduce the level of gastrin and promote the repair of gastric mucosa.It has high safety and is worthy to be popularized in clinic.
