基于体质辨识理论的护理结合穴位敷贴在消化性溃疡伴出血中的应用效果

Application effect of nursing based on constitution identification theory combined with acupoint application in peptic ulcer with bleeding

目的探讨基于体质辨识理论的护理结合上消化道止血穴位敷贴在消化性溃疡伴出血患者中的应用效果。方法选取贺州市人民医院2021年1~12月收治的112例消化性溃疡伴出血患者为研究对象,按随机信封法分为对照组和观察组,各56例。对照组给予西药奥美拉唑治疗与常规护理干预,观察组在对照组护理方法的基础上对患者的体质进行辨识,并根据不同证型选取不同穴位采用上消化道止血贴进行干预。比较2组的中医证候积分、血常规指标、并发症发生情况和生活质量。结果出院前1 d,2组的胃疼、呕血和黑便主证积分均低于入院时,消化病生存质量指数各维度得分均高于入院时,且观察组的上述评分均优于对照组,差异均有统计学意义(P均<0.05);出院前1 d及干预第3天,2组的血红蛋白、血细胞比容和红细胞水平均高于入院时,且观察组均高于同期对照组,差异均有统计学意义(P均<0.05)。结论基于体质辨识理论的护理结合上消化道止血穴位敷贴应用于消化性溃疡伴出血患者,可改善血常规指标水平及中医证候积分情况,提升生活质量。

Objective To explore the application effect of nursing based on constitution identification theory combined with upper gastrointestinal hemostatic acupoint application in patients with peptic ulcer bleeding.Methods A total of 112 patients with peptic ulcer bleeding admitted to Hezhou People's Hospital from January to December 2021 were recruited in the study.They were divided into the control group and the observation group via envelope randomization,with 56 cases in each group.The control group was treated with western medicine omeprazole and routine nursing intervention.In the observation group,the constitution of the patients was identified on the basis of the nursing method of the control group.Different acupoints were selected according to different syndromes,and the upper gastrointestinal hemostatic acupoint applications were used.The traditional Chinese medicine syndrome scores,blood routine parameters,complications,and quality of life of the two groups were compared.Results At 1 day before discharge,the main symptom scores of stomach pain,hematemesis,and melena in the two groups were lower than those at admission,and the scores of all dimensions of the Gastrointestinal Quality of Life Index were higher than those at admission(all P<0.05).The above scores in the observation group were better than those in the control group(all P<0.05).At 1 day before discharge and after 3 days of intervention,the levels of hemoglobin,hematocrit,and erythrocyte count in the two groups were higher than those at admission,and the levels in the observation group were higher than those in the control group(all P<0.05).Conclusion Nursing based on the constitution identification theory combined with upper gastrointestinal hemostatic acupoint application can improve the blood routine parameters,traditional Chinese medicine syndrome scores,and quality of life of patients with peptic ulcer bleeding.
