n-ha/PA66 Cage生物活性材料的应用比较及效果分析

摘要 以纳米羟基磷灰石/聚酰胺66椎间融合器(n-ha/PA66 Cage)为例探讨活性纳米生物材料椎间融合器用于治疗腰椎退行性疾病的临床价值及其护理措施。采用n-HA/PA66 Cage行椎间植骨融合内固定术治疗的100例腰椎退行性变性患者,分为单节段组和双节段组,其中单节段组45例,双节段融合55例。比较2组围术期相关指标以及影像学评价结果。结果表明:2组患者在手术出血量、手术时长、术后引流,以及总住院时间相近(P>0.05)。影像学检测显示2组术后融合节段椎间隙高度较术前有效提高(P<0.05);术后3 d椎间隙高度相近(P>0.05),术后3个月双节段组椎间隙高度高于单节段组(P<0.05);2组术后3 d、3个月及末次随访时n-ha/PA66 Cage的植入深度相似(P>0.05);2组均随时间延长而植入的深度明显改变(P>0.05);2组患者在末次随访均实现骨性融合,改良ODI和VAS评分较基线下降(P<0.05);双节段组在VAS评分上劣于单节段组(P<0.05);其余各时间点VAS评分、改良ODI组间比较无差异(P>0.05)。说明n-HA/PA66材质的椎间融合器能有效重建并维持腰椎退变性侧凸患者融合节段椎间隙高度,增加脊柱稳定性,对双阶段患者效果更佳。 Taking nano hydroxyapatite/polyamide 66 interbody fusion cage(n-ha/PA66 Cage)as an example,to explore the clinical value and nursing measures of active nano biomaterial interbody fusion cage in the treatment of lumbar degenerative diseases.100 patients with lumbar degenerative degeneration were treated with n-HA/PA66 Cage interbody fusion and internal fixation.They were divided into single segment group and double segment group,including 45 patients in single segment group and 55 patients in double segment fusion group.The perioperative related indexes and imaging evaluation results were compared between the two groups.The results show that the bleeding volume,operation duration,postoperative drainage and total hospital stay of the two groups were similar(P>0.05).Imaging examination show that the height of intervertebral space at fusion segment in both groups was significantly higher than that before operation(P<0.05);the height of intervertebral space was similar 3 days after operation(P>0.05),and that of the double segment group was higher than that of the single segment group 3 months after operation(P<0.05);the implantation depth of n-ha/PA66 Cage was similar in the two groups 3 days,3 months after operation and at the last follow-up(P>0.05);the depth of implantation in both groups changed significantly with time(P>0.05);the patients in both groups achieved bone fusion at the last follow-up,and the improved ODI and VAS scores decreased compared with the baseline(P<0.05);the VAS score of the two segment group was inferior to that of the single segment group(P<0.05);there was no difference in VAS score and improved ODI at other time points(P>0.05).It shows that n-HA/PA66 interbody fusion cage can effectively reconstruct and maintain the height of intervertebral space at the fusion segment of patients with lumbar degenerative scoliosis,increase the stability of the spine,and has better effect on patients with two-stage lumbar degenerative scoliosis.
