导乐联合无痛分娩对初产妇自我效能及总产程的影响

Effect of Doule Combined with Painless Delivery on Self-efficacy and Total Labor Process of Primipara

目的分析导乐联合无痛分娩对初产妇自我效能及总产程的影响。方法回顾性分析医院2019年1月至2020年1月接收的230例初产妇临床资料,根据其分娩干预方式分为参照组(110例)与研究组(120例),参照组实施常规分娩干预,研究组实施导乐联合无痛分娩干预。比较两组不同产程现有疼痛强度评定(PPI)评分、产程、分娩结局,并比较两组干预前后一般自我效能感量表(GSES)评分、焦虑自评量表(SAS)评分。结果研究组第一产程、第二产程、第三产程PPI评分低于参照组,差异有统计学意义(P<0.05)。干预后,研究组GSES评分为(30.41±4.82)分,高于参照组的(28.33±4.57)分,差异有统计学意义(P<0.05)。干预后,研究组SAS评分低于参照组,差异有统计学意义(P<0.05)。研究组第一产程、第二产程、总产程时间短于参照组,差异有统计学意义(P<0.05)。研究组不良分娩结局发生率为2.50%,低于参照组的10.00%,差异有统计学意义(P<0.05)。结论采用导乐联合无痛分娩可有效缓解初产妇焦虑情绪,增强其自我效能感,减轻疼痛,积极配合分娩,缩短产程,减少不良分娩结局的发生。

Objective To analyze the effect of doule combined with painless delivery on the self-efficacy and total labor process of primipara.Methods The clinical data of 230 primiparas received by the hospital from January 2019 to January 2020 were retrospectively analyzed.According to their delivery intervention methods,they were divided into the reference group(110 cases)and the study group(120 cases).The reference group implemented routine delivery intervention,and the study group implemented doule combined with painless delivery intervention.The existing pain intensity assessment(PPI)score,labor process and labor outcome were compared between the two groups in different stages of labor,and the scores of General Self-Efficacy Scale(GSES)and Self-rating Anxiety Scale(SAS)were compared between the two groups before and after the intervention.Results The PPI scores of the first stage of labor,the second stage of labor and the third stage of labor in the study group were lower than those in the reference group(P<0.05).After the intervention,the GSES score of the study group was(30.41±4.82),which was higher than that of the reference group(28.33±4.57),P<0.05.After the intervention,the SAS score of the study group was lower than that of the reference group,P<0.05.The first stage of labor,the second stage of labor and the total stage of labor in the study group were shorter than those in the reference group(P<0.05).The incidence of adverse delivery outcomes in the study group was 2.50%,lower than that in the reference group(10.00%)(P<0.05).Conclusion Doule combined with painless delivery can effectively relieve the anxiety of primiparas,enhance their self-efficacy,reduce pain,actively cooperate with labor,shorten the labor process,and reduce the occurrence of adverse labor outcomes.
