小剂量尿激酶联合氯吡格雷治疗超溶栓时间窗急性脑梗死老年患者的疗效观察

Observation on Efficacy of Small-Dose Urokinase Combined with Clopidogrel in Treatment of Elderly Patients with Acute Cerebral Infarction Extending Thrombolytic Time Window

目的:分析小剂量尿激酶联合氯吡格雷治疗超溶栓时间窗急性脑梗死老年患者的临床效果。方法:选取靖江市第二人民医院2019年9月-2021年9月收治的48例超溶栓时间窗急性脑梗死老年患者,根据随机数字表法分为两组,各24例。对照组采用小剂量尿激酶治疗,观察组采用小剂量尿激酶联合氯吡格雷治疗。比较两组患者治疗前后神经功能、运动功能、凝血指标以及治疗后侧支循环开放情况。结果:观察组治疗后神经功能评分低于对照组,运动功能评分高于对照组,两组差异有统计学意义(P<0.05)。观察组治疗后各项凝血指标情况均优于对照组,差异有统计学意义(P<0.05)。观察组治疗后侧支循环开放情况高于对照组,差异有统计学意义(P<0.05)。结论:小剂量尿激酶联合氯吡格雷治疗超溶栓时间窗急性脑梗死老年患者的效果显著,不仅可以改善神经及运动功能,提高侧支循环开放率,还能纠正其凝血功能。

Objective:To analyze the clinical effect of small-dose urokinase combined with clopidogrel in the treatment of acute cerebral infarction in elderly patients extending thrombolytic time window.Methods:Forty-eight elderly patients with acute cerebral infarction extending thrombolytic time window who treated in Jingjiang Second People's Hospital from September 2019 to September 2021 were selected and divided into two groups according to random digital number table method,with 24 cases in each group.The control group was treated with small-dose urokinase,and the observation group was treated with small-dose urokinase combined with clopidogrel.The neurological function,motor function,coagulation index before and after treatment and the opening rate of collateral circulation after treatment were compared in the two groups.Results:After treatment,the neurological function score of the observation group was lower than that of the control group,and the motor function score was higher than that of the control group,and the difference between the two groups was statistically significant(P<0.05).After treatment,the coagulation indexes of the observation group were better than those of the control group,and the difference was statistically significant(P<0.05).The opening of collateral circulation of the observation group was higher than that of the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion:Small-dose urokinase combined with clopidogrel has a significant effect on the treatment of acute cerebral infarction in elderly patients extending thrombolytic time window.It can not only improve the neurological and motor functions,increase the collateral circulation opening rates,but also correct their coagulation function.
