麦默通微创旋切术治疗乳腺肿物的效果分析

Effect Analysis of Mammotome Minimally Invasive Atherectomy in Treatment of Breast Tumors

目的:探究乳腺肿物治疗中麦默通微创旋切术的应用效果。方法:选取2020年1月-2022年1月湖南省人民医院马王堆院区乳腺微创旋切的149例患者,共185个乳腺肿物实施麦默通微创旋切术,患者来源于湖南省人民医院普外五科或乳甲外二科。分析治疗前后患者负面情绪、生活质量情况、术后结果、并发症发生率以及术后3个月随访复查情况。结果:149例患者共185个乳腺肿物中,腺病结节占比最多为45.95%(85/185),其次为纤维腺瘤32.43%(60/185);术后皮肤瘀斑发生率为10.74%(16/149)、局部血肿发生率为2.01%(3/149)、局部出血发生率为12.75%(19/149),术后感染发生率为0;术后3个月随访复查,所有患者局部血肿、皮肤瘀斑均完全吸收,超声复查后发现有2例(1.34%)在原手术位置存在超声影像边界不清的低回声肿物,再次行超声引导下组织穿刺活检,病理结果显示均为良性改变,未再次手术,指导其定期复查。结论:超声引导下行麦默通微创旋切术在治疗乳腺肿物中有着较好的效果,是传统手术方式的变革,值得推广。

Objective: To explore the application effect of Mammotome minimally invasive atherectomy in the treatment of breast tumors. Methods: From January 2020 to January 2022, 149 patients with a total of 185 breast tumors undergoing Mammotome minimally invasive breast atherectomy in the Mawangdui District of Hunan Provincial People’s Hospital were selected. The patients came from the Fifth Department of General Surgery or the Second Department of Breast and Thyroid Surgery in Hunan Provincial People’s Hospital. The negative emotions and quality of life before and after treatment, postoperative outcomes,complication rates, and postoperative 3-month follow-up review were analyzed. Results: Among 149 patients, there were a total of185 breast tumors, among which adenopathy nodules accounted for the largest proportion(45.95%, 85/185), followed by fibroadenoma(32.43%, 60/185). The incidence of postoperative skin ecchymosis was 10.74%(16/149), the incidence of local hematoma was 2.01%(3/149), the incidence of local bleeding was 12.75%(19/149), and the incidence of postoperative infection was 0. After 3 months of follow-up and review, all patients’ local hematoma and skin ecchymosis were completely absorbed. After ultrasound review, 2 cases(1.34%) had hypoechoic mass with unclear boundary at the original surgical site. After ultrasoundguided tissue puncture biopsy, pathological results showed benign changes, no reoperation was performed, and regular review was instructed. Conclusion: Ultrasound-guided Mammotome minimally invasive atherectomy has a good effect in the treatment of breast tumors. It is an innovation from traditional surgical methods and is worthy of promotion.
