单级电凝扁桃体切除术与低温等离子切除术在扁桃体切除术患者中的疗效比较

Comparison of the efficacy of monopole electro coagulation tonsillectomy and low-temperature plasma resection in patients undergoing tonsillectomy

目的对比单极电凝扁桃体切除术与低温等离子切除术在扁桃体切除术患者中的疗效差异。方法选取2020年1月至2021年1月我院实施扁桃体切除术患者68例,随机分组各34例,对照组行单极电凝扁桃体切除术治疗,观察组行低温等离子切除术法治疗,观察临床疗效,详细记录两组手术相关指标,应用视觉模拟评分法(VAS)评估两组手术治疗后1 d、3 d、7 d疼痛状况,检测两组手术治疗前后血清肿瘤坏死因子-α(TNF-α)、超敏-C反应蛋白(hs-CRP)以及白细胞介素-6(IL-6)等炎性细胞因子水平变化,统计术后并发症发生情况。结果观察组临床总有效率较对照组高(P<0.05);观察组手术治疗时间、伪膜脱落时间、白膜形成时间、恢复饮食时间、住院时间较对照组短,术中出血量较对照组低(P<0.05);观察组术后1 d、3 d、7 d疼痛VAS评分较对照组低(P<0.05);治疗前,两组TNF-α、hs-CRP、IL-6水平差异无统计学意义(P>0.05),治疗后,观察组TNF-α、hs-CRP、IL-6水平较对照组低(P<0.05);观察组术后并发症发生率较对照组低(P<0.05)。结论对我院接受扁桃体切除术患者予以低温等离子切除术法治疗,相较于单极电凝扁桃体切除术疗效更佳,不仅可以缩短手术治疗时间,还能有效减轻术后疼痛感,避免机体内炎性细胞因子过量分泌,从而防止术后并发症发生,促进临床疗效提升,有效性、安全性较高。

Objective To compare the difference in efficacy between monopole electro coagulation tonsillectomy and low-temperature plasma resection in patients undergoing tonsillectomy.Methods A total of 68 patients with tonsillectomy in our hospital from January 2020 to January 2021 were selected and randomly divided into two groups,34 cases in each group.The control group was treated with monopole electro coagulation tonsillectomy,while the observation group was treated with low-temperature plasma resection.The clinical efficacy was observed,and the surgeryrelated indicators of the two groups were recorded in detail.Visual analogue scale(VAS)was used to evaluate the pain status of the two groups at 1,3 and 7 days after operation.The changes of serum tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6)and other inflammatory cytokines before and after operation were detected in the two groups.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).The operation treatment time,pseudomembrane shedding time,albuginea formation time,diet recovery time and hospitalization time in the observation group were shorter than those in the control group,and the intraoperative blood loss was lower than that in the control group(P<0.05).The VAS scores on postoperative day 1,3 and 7 in the observation group were lower than those in the control group(P<0.05).Before treatment,there were no significant differences in the levels of TNF-α,hs-CRP and IL-6 between the two groups(P>0.05),but after treatment,the levels of TNF-α,hs-CRP and IL-6 in the observation group were lower than those in the control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion In our hospital in patients with tonsillectomy by low-temperature plasma resection method of treatment,compared with monopole electric coagulation tonsillectomy curative effect is better,can not only shorten the surgery time,also can effectively reduce postoperative pain,avoid the inflammatory cytokine secretion in the body and preventing postoperative complications and promote the clinical curative effect,high efficacy,safety.
