腹腔镜切除胆囊与小切口胆囊切除治疗胆结石的临床疗效观察

To Observe the Clinical Effect of Laparoscopic Cholecystectomy and Small Incision Cholecystectomy in the Treatment of Gallstones

目的探讨胆囊结石患者行腹腔镜切除胆囊与小切口胆囊切除的临床效果。方法选择本院于2019年2月至2020年2月收治的胆囊结石患者117例,按照随机数字表法分为两组,对照组(57例,采用小切口胆囊切除术治疗)和观察组(60例,采用腹腔镜切除胆囊治疗),收集分析两组患者手术指标、炎性因子水平、免疫功能水平以及并发症发生情况。结果观察组患者术中出血量、肛门排气时间、术后VAS评分、住院时间明显低于对照组(P<0.05);术前两组患者炎性因子水平、免疫功能比较无明显差异(P>0.05),术后2 d观察组CRP、IL-6水平明显低于对照组,IgA、IgM、IgG含量明显高于对照组(P<0.05);观察组并发症发生率明显低于对照组(6.67%vs.24.56%)(P<0.05)。结论针对胆结石患者临床可采用腹腔镜切除胆囊术治疗,患者术后恢复较快,并发症明显降低。

Objective To explore the effect of laparoscopic cholecystectomy and small incision cholecystectomy in patients with gallstone.Methods One hundred and seventeen patients with cholecystolithiasis admitted to our hospital from February 2019 to February 2020 were divided into two groups according to the random number table.The control group(57 cases,treated with small incision cholecystectomy)and the observation group(60 cases,treated with laparoscopic cholecystectomy).The operation indexes,inflammatory factor level,immune function level and complications of the two groups were collected and analyzed condition.Results The amount of intraoperative bleeding,anal exhaust time,postoperative VAS score and hospitalization time in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the level of inflammatory factors and immune function between the two groups before operation(P>0.05);two days after surgery,the levels of CRP and IL-6 in the observation group were significantly lower than those in the control group,IgA,IgM and IgG was significantly higher than that of the control group(P<0.05);the complications rate of the observation group was significantly lower than that of the control group(6.67%vs.24.56%)(P<0.05).Conclusion Laparoscopic cholecystectomy can be used in the treatment of cholelithiasis.The recovery of patients is faster and the complications are significantly reduced.
