经阴道超声评估PCOS的影像学价值分析

Analysis of the imaging value of transvaginal ultrasonography in evaluating PCOS

目的 对多囊卵巢综合征(PCOS)患者应用经阴道超声技术评估的影像学价值进行分析。方法 选取90例疑似PCOS患者,以临床诊断结果为金标准,将36例PCOS患者列为A组,30例卵泡膜细胞增殖综合征列为B组,24例卵巢细胞肿瘤列为C组,比较各项指标。结果 阴道超声对PCOS诊断的准确度为90.00%,敏感度为83.33%,特异度为94.44%;与B组、C组进行比较,A组的卵巢体积较大,卵巢数目较多,卵泡直径均值较长,且病灶边缘光滑、包膜回声及被膜增厚回声增强、Adler分级Ⅱ~Ⅲ级、条状血流的患者占比均较高,卵巢间质动脉阻力指数(RI)、动脉搏动指数(PI)均较低,差异具有统计学意义(P<0.05)。结论 经阴道超声检查能够对PCOS患者进行有效诊断,PCOS患者的影像学特征与卵泡膜细胞增殖综合征、卵巢细胞肿瘤患者存在明显区别,结合超声影像特征可有效观测患者卵巢情况,可辅助临床确诊。

Objective To analyze the imaging value of transvaginal ultrasound in patients with polycystic ovary syndrome(PCOS). Methods 90 patients with suspected PCOS were selected. Clinical diagnosis was the gold standard, 36 patients with PCOS were classified as group A, 30 patients with theca cell proliferation syndrome were classified as group B, and 24 patients with ovarian cell tumors were classified as group C. The indicators were compared. Results The accuracy of vaginal ultrasound in the diagnosis of PCOS was 90.00%, the sensitivity was 83.33%, and the specificity was 94.44%;compared with group B and group C,group A had larger ovary volume, more ovaries, longer average follicle diameter, smooth lesion edge, enhanced echogenic capsule and thickened capsule, the proportion of grades Ⅱ to Ⅲ of Adler classifi cation and strip-shaped blood flow were higher, and the PI and RI of ovarian interstitial arteries were lower, the difference was statistically significant(P<0.05). Conclusion Transvaginal ultrasonography can effectively diagnose PCOS patients. The imaging features of PCOS patients are significantly different from those of theca cell proliferation syndrome and ovarian cell tumors. The combination of ultrasonography features can effectively observe the ovarian condition of patients and assist clinical diagnosis.
