隆乳假体相关乳腺肿瘤的研究进展

Research progress of breast tumor associated with breast prosthesis

近年来,以假体为基础的乳房重建术蓬勃发展,乳房假体植入后乳房相关肿瘤日益多发,尤其是乳房假体相关间变性大细胞淋巴瘤(BIA-ALCL)发生率逐步增高,主要临床表现为伴血清肿的乳房肿胀、乳房肿物及淋巴结肿大。发病机制尚不明确,可能与慢性炎症刺激、毛面假体诱发、免疫反应及某些病原体感染有关。常用的诊断方法有乳腺超声检查、假体周围渗液的细胞学检查、免疫表型检查及乳房肿块的组织活检。治疗方法主要依靠手术治疗。本文对乳房假体植入后发生相关肿瘤的文献进行了系统回顾和综述,以期引起临床医生的警惕和思考。

In recent years, breast reconstruction based on prosthesis has developed vigorously, breast related tumors are increasingly common after breast prosthesis implantation, especially the incidence of breast prosthesis related anaplastic large cell lymphoma(BIA-ALCL) is gradually increasing, the main clinical manifestations are breast swelling with seroma, breast mass and lymphadenopathy. The pathogenesis is not clear, and may be related to chronic inflammatory stimulation, hairy prosthesis induction, immune response and certain pathogenic infections. The common diagnostic methods include breast ultrasound,cytological examination of periprosthetic exudate, immunophenotypic examination and tissue biopsy of breast mass. The treatment method mainly depends on surgical treatment. This paper carries out a systematic review and overview of the literature on the occurrence of related tumors after breast implantation in order to arouse the vigilance and thinking of clinicians.
