不同类型宫颈癌筛查方法对本地人群适宜研究

Study on the suitability of different screening methods for cervical cancer in local population

目的 对不同类型宫颈癌筛查方法进行研究。方法 通过选取2017年6月~2019年5月收治的行宫颈癌筛查并行阴道镜下活检的女性150例,通过以组织病理学作为金标准,并依照结果进行分组,分别采用液基细胞学(TCT)+人乳头瘤病毒(HPV)(C组)、TCT(B组)、醋酸染色法(VTA)(A组)三种不同的方式做好研究工作。结果 通过对宫颈癌筛查方式构成进行研究,其中宫颈癌筛查中选用几种方式包含:TCT+VTA、TCT、VTA分别为30例、49例、71例,所占比例为13.00%、32.67%、54.00%,选择VTA人群居多。通过对宫颈癌筛查中阳性率、灵敏度进行比较,其中采用TCT+VTA、TCT、VTA灵敏度分别为89.0%、78.0%、75.0%,特异度为75.0%、65.9%、64.3%。通过对三组宫颈癌筛查患者进行分期比较,其中TCT+VTA、TCT、VTA的病理学中阳性率分别为10.67%、8.67%、7.33%。结论 在宫颈癌筛查中选用TCT+VTA阳性率和灵敏度更高,为优选筛查方法;受成本影响,自愿选择VTA比例相对较多,因此对于不同人群来说选用不同的方式进行筛查。

Objective To study the screening Methods for different types of cervical cancer. Methods A total of 150women who underwent cervical cancer screening and colposcopy biopsy were selected from June 2017 to May 2019. By taking histopathology as the gold standard and grouping according to the Results, liquid-based cytology(TCT) + human papillomavirus(HPV)(group C), TCT(group B), VTA(group A) three different ways to do research work. Results through the study on the composition of cervical cancer screening methods, several Methods were selected in cervical cancer screening, including: TCT +VTA, TCT and VTA were 30 cases, 49 cases and 71 cases respectively, accounting for 13.00%, 32.67% and 54.00%, and most people chose VTA. By comparing the positive rate and sensitivity in cervical cancer screening, the sensitivity of TCT + VTA, TCT and VTA were 89.0%, 78.0% and 75.0% respectively, and the specificity was 75.0%, 65.9% and 64.3%. By comparing the three groups of cervical cancer screening patients by stages, the positive rates of TCT + VTA, TCT and VTA were 10.67%, 8.67% and 7.33%respectively. Conclusion TCT + VTA has higher positive rate and sensitivity in cervical cancer screening, which is the optimal screening method;Affected by the cost, the proportion of voluntary choice of VTA is relatively large, so different screening Methods are selected for different populations.
