绒促性素联合粒细胞集落刺激因子宫内灌注对薄型子宫内膜不孕患者妊娠的影响

Effect of intrauterine infusion of HCG combined with granulocyte colony stimulating factor on pregnancy in patients with thin endometrial infertility

目的 探析绒促性素联合粒细胞集落刺激因子宫内灌注对薄型子宫内膜不孕患者妊娠的影响。方法 选择2019年1月-2020年5月,本院收治的32例薄型子宫内膜不孕患者为研究对象,按随机数字表法,分为对照组(16例)和观察组(16例)。对照组行绒促性素肌内注射治疗+生理盐水灌注治疗,观察组行绒促性素肌内注射治疗+粒细胞集落刺激因子宫内灌注治疗。比较两组的子宫内膜改善情况、不良反应及妊娠情况。结果 观察组治疗后5d、10d的子宫内膜厚度大于对照组,治疗后10d观察组内膜容积≥2mL、内膜下血流为Ⅲ型的患者人数多于对照组,差异有统计学意义(P<0.05)。比较两组的内膜类型、不良反应总发生率、移植胚胎数、优质胚胎率、胚胎种植率、生化妊娠率、临床妊娠率,差异无统计学意义(P>0.05)。结论 将绒促性素联合粒细胞集落刺激因子宫内灌注治疗方式应用到薄型子宫内膜不孕的临床治疗中,可达较理想的用药效果,能增加子宫内膜厚度,改善子宫内膜状态,用药安全度较高,不会对胚胎质量造成过多影响,具有一定推广应用价值。

Objective To investigate the effect of intrauterine infusion of HCG combined with granulocyte colony stimulating factor on pregnancy in patients with thin endometrial infertility. Methods Thirty-two patients with thin endometrial infertility admitted to our hospital from January 2019 to May 2020 were selected as the research objects, and divided into control group(16cases) and observation group(16 cases) according to the random number table method. The control group received intramuscular injection of HCG + saline infusion, while the observation group received intramuscular injection of HCG + granulocyte-colonystimulating factor intrauterine infusion. Endometrial improvement, adverse reactions and pregnancy were compared between the two groups. Results The endometrial thickness of the observation group was greater than that of the control group at 5 and 10 days after treatment, and the number of patients with endometrial volume ≥2mL and subintimal blood flow type Ⅲ in the observation group was more than that in the control group at 10 days after treatment, with statistically significant differences(P<0.05). There were no significant differences in endometrial type, total incidence of adverse reactions, number of transplanted embryos, rate of high-quality embryos, embryo implantation rate, biochemical pregnancy rate and clinical pregnancy rate between the two groups(P>0.05).Conclusion The application of chorionic gonadotropin combined with granulocyte colony-stimulating factor intrauterine perfusion therapy in the clinical treatment of thin endometrial infertility can achieve ideal drug effect, increase endometrial thickness, improve endometrial status, have a high degree of safety, and will not have too much impact on embryo quality,It has certain popularization and application value.
