彩色多普勒超声对子宫肌瘤与子宫腺肌瘤的鉴别诊断

Differential Diagnosis of Uterine Fibroids and Uterine Adenomyoma by color doppler ultrasound

目的探讨分析彩色多普勒超声对子宫肌瘤与子宫腺肌瘤的鉴别诊断价值。方法选取57例接受超声检查的妇科疾病患者(包括36例子宫肌瘤患者,21例子宫腺肌瘤患者),采用彩色多普勒超声进行检查,以手术病理诊断为参考,统计彩色多普勒超声对子宫肌瘤患者与子宫腺肌瘤患者的诊断符合率,并对比血流灌注指数、舒张期血流峰值及子宫平均阻力指数。结果彩色多普勒超声检查结果显示,子宫肌瘤组中检出例数为35例,诊断符合率为97.2%;子宫腺肌瘤组中检出例数为21例,诊断符合率为100%,差异无统计学意义(P>0.05)。经过统计分析发现,子宫肌瘤组患者血流灌注指数明显高于子宫腺肌瘤组,而子宫腺肌瘤组患者舒张期血流峰值以及子宫平均阻力指数明显高于子宫肌瘤组患者,差异具有统计学意义(P<0.05)。结论彩色多普勒超声是临床对子宫肌瘤和子宫腺肌瘤患者进行鉴别诊断的重要手段,其诊断符合率较高,且在子宫血流动力学指标方面有明显的差异化表现,具有重要的参考价值。

Objective To explore and analyze the value of color doppler ultrasonography in the differential diagnosis of uterine fibroids and adenomyoma.Methods 57 patients with gynecological diseases(including 36 patients with uterine fibroids and 21 patients with adenomyoma)who underwent ultrasonography were selected as observation objects,and color Doppler ultrasonography was used to examine the enrolled objects.For reference,the diagnostic coincidence rate of color Doppler ultrasound in patients with uterine fibroids and patients with adenomyoma was calculated,and the blood perfusion index,peak diastolic blood flow and average uterine resistance index were compared.Results The results of color doppler ultrasonography showed that there were 35 cases detected in the uterine fibroids group,and the diagnostic coincidence rate was 97.2%;21 cases were detected in the uterine adenomyoma group,and the diagnostic coincidence rate was 100%,the difference was not statistically significant(P>0.05).After statistical analysis,it was found that the blood perfusion index of the uterine fibroids group was significantly higher than that of the adenomyoma group,while the peak diastolic blood flow of the uterine fibroids group was significantly higher.And the difference was statistically significant(P<0.05).Conclusion Color doppler ultrasonography is an important method for differential diagnosis of patients with uterine fibroids and adenomyoma,and its diagnostic coincidence rate High,and there are obvious differences in uterine hemodynamic indicators,which has important reference value.
