三维能量多普勒超声在子宫内膜息肉诊断与鉴别诊断中的应用价值

Application value of three-dimensional power Doppler ultrasonography in the diagnosis and differential diagnosis of endometrial polyps

目的探讨三维能量多普勒超声(3D-PDUS)在子宫内膜息肉(EP)诊断与鉴别诊断中的应用价值。方法选取2020年1月至2021年1月在泰州市中医院进行超声检查的86例患者为研究对象。所有患者经宫腔镜检查、病理检验确诊,其中EP患者40例(内膜息肉组),子宫内膜增生46例(内膜增生组)。所有患者接受3D-PDUS检查,比较各组3D-PDUS血流参数,包括内膜体积,内膜内血管指数(VI1)、血流指数(FI1)、血管血流指数(VFI1),内膜周边2 mm的VI(VI2)、FI(FI2)、VFI(VFI2)。结果内膜息肉组平均内膜体积为(6.55±1.89)cm^(3),内膜增生组为(9.72±2.02)cm^(3),对照组为(3.62±1.88)cm^(3),差异有统计学意义(P<0.05);内膜息肉组内膜内VI1、FI1、VFI1高于内膜增生组、对照组,差异有统计学意义(P<0.05);内膜息肉组VI2、VFI2低于内膜增生组,VI2、FI2、VFI2高于对照组,差异有统计学意义(P<0.05)。结论3D-PDUS在诊断EP中具有较高的价值,可获取相关血流参数,提高EP与子宫内膜增生的鉴别诊断效果,值得推广。

Objective To explore the value of three-dimensional power Doppler ultrasound(3D-PDUS)in the diagnosis and differential diagnosis of endometrial polyps(EP).Methods A total of 86 patients with ultrasound examination from January 2020 to January 2021 in Taizhou City Hospital of Traditional Chinese Medicine were selected as the research objects.All the patients were confirmed by hysteroscopy and pathological examination,including 40 patients with EP(endometrial polyps group)and 46 patients with endometrial hyperplasia(endometrial hyperplasia group).All the patients received 3D-PDUS examination,and the blood flow parameters of 3D-PDUS in each group were compared,including intimal volume,intimal vascular index(VI1),blood flow index(FI1)and vascular blood flow index(VFI1),and VI2,FI2,VFI2 at 2 mm around the intima.Results The average intimal volume was(6.55±1.89)cm^(3) in the endometrial polyps group,(9.72±2.02)cm^(3) in the endometrial hyperplasia group and(3.62±1.88)cm^(3) in the control group,and there were significant differences among three groups(P<0.05);VI1,FI1 and VFI1 in the endometrial polyps group were significantly higher than those in the endometrial hyperplasia group and the control group(P<0.05);VI2 and VFI2 in the endometrial polyps group were significantly lower than those in the endometrial hyperplasia group,and VI2,FI2 and VFI2 were significantly higher than those in the control group(P<0.05).Conclusion 3D-PDUS shows a high value in the diagnosis of EP,which can obtain relevant blood flow parameters and improve the differential diagnosis effect between EP and endometrial hyperplasia,so it is worthy of popularization.
