产科急症剖宫产与择期剖宫产的临床特征对照分析

Comparative analysis of clinical characteristics between obstetric emergency cesarean section and elective cesarean section

目的对照分析产科急症剖宫产与择期剖宫产的临床特征。方法选取我院120例进行回顾性分析。60例纳入急症组,60例纳入择期组。对两组手术指征、临床指标以及不良事件发生情况进行观察、统计与比较。结果急症组剖宫产手术指征总发生率高于对照组,疤痕子宫、骨盆狭窄、胎儿宫内窘迫、宫缩乏力、脐带脱垂、胎位异常、产程停滞等占比比较,差异均有统计学意义(P<0.05);择期组手术时间、术中出血量、肛门首次排期时间等临床指标水平均低于急症组,差异有统计学意义(P<0.05);急症组不良事件发生率明显高于对照组(P<0.05)。结论急症剖宫产手术指征现象明显高于择期剖宫产,具有较高术中与术后风险,需给予高度重视,准确把握手术指征,做好各项防护。

Objective The clinical characteristics of obstetric emergency cesarean section and elective cesarean section were compared and analyzed.Methods 120 cases in our hospital were analyzed retrospectively.60 cases were included in the emergency group and 60 cases in the elective group.The surgical indications,clinical indexes and adverse events of the two groups were observed,counted and compared.Results the total incidence of cesarean section indications in the emergency group was higher than that in the control group.There were significant differences in the proportions of scar uterus,pelvic stenosis,fetal distress,uterine weakness,umbilical cord prolapse,abnormal fetal position and labor Stagnation(P<0.05);The levels of clinical indexes such as operation time,intraoperative bleeding and the first anal scheduling time in the selective group were lower than those in the emergency group(P<0.05);The incidence of adverse events in the emergency group was significantly higher than that in the control group(P<0.05).Conclusion the indication phenomenon of emergency cesarean section is significantly higher than that of elective cesarean section,which has higher intraoperative and postoperative risks.Great attention should be paid to accurately grasp the operation indication and do a good job in various protection.
