经脐单孔腹腔镜手术治疗不同直径及类型卵巢囊肿的可行性研究

Feasibility of Transumbilical Laparoendoscopic Single-site Surgery in the Treatment of Ovarian Cysts of Different Diameters and Types

目的探讨经脐单孔腹腔镜手术(TU-LESS)治疗不同直径及类型卵巢囊肿的可行性。方法回顾性分析2018年3月—2021年5月收治的行TU-LESS治疗的卵巢囊肿98例的临床资料,统计所有患者囊肿直径和类型,比较不同直径和类型卵巢囊肿患者相关指标。结果98例卵巢囊肿中,囊肿直径<10 cm 62例,囊肿直径≥10 cm 36例;囊腺瘤52例,子宫内膜异位囊肿26例,囊性畸胎瘤20例。囊肿直径<10 cm和≥10 cm患者术中出血量、术后肛门排气时间、术后住院天数、术后视觉模拟评分法评分及术后血清雌激素、抗苗勒管激素、卵泡刺激素水平比较差异均无统计学意义(P>0.05)。囊肿直径<10 cm患者手术时间短于囊肿直径≥10 cm患者(P<0.01)。不同类型卵巢囊肿患者上述相关指标比较差异均无统计学意义(P>0.05)。结论TU-LESS治疗不同直径及类型卵巢囊肿均可行。

Objective To investigate the feasibility of transumbilical laparoendoscopic single-site surgery(TU-LESS)in the treatment of ovarian cysts of different diameters and types.Methods The clinical data of 98 patients with ovarian cysts who underwent TU-LESS in the hospital from March 2018 to May 2021 were retrospectively collected.Diameters and types of cysts in all patients were calculated.Relevant indicators were compared between patients with different diameters and types of ovarian cysts.Results Among the 98 patients with ovarian cysts,there were 62 patients with cysts<10 cm,36 patients with cysts≥10 cm,52 patients with cystadenoma,26 patients with endometriosis cysts,and 20 patients with cystic teratoma.The intraoperative blood loss,postoperative anal exhaust time,length of postoperative hospital stay,postoperative Visual Analog Scale(VAS)score and postoperative levels of serum estrogen,anti-Müllerian hormone(AMH)and follicle-stimulating hormone(FSH)showed no statistically significant difference between the groups(P>0.05).The duration of operation of patients with cysts<10 cm was shorter than that of patients with cysts≥10 cm(P<0.01).The above-mentioned indicators showed no significant differences in patients with different types of ovarian cysts(P>0.05).Conclusion TU-LESS is feasible for patients with ovarian cysts of different diameters and types.
