枯草杆菌二联活菌颗粒联合阿莫西林-克拉维酸钾治疗新生儿肺炎有效性及对康复进程、炎性指标的影响 被引量:7

Efficacy of Live Combined Bacillus Subtilis and Enterococcus Faecium Granules With Multivitamines Combined with Amoxicillin-clavulanate Potassium in the Treatment of Neonatal Pneumonia and Its Effects on the Recovery Process and Inflammatory Indicators

摘要 目的探讨枯草杆菌二联活菌颗粒联合阿莫西林-克拉维酸钾治疗新生儿肺炎的有效性及对康复进程、炎性指标的影响。方法选取2020年12月—2022年5月收治的78例新生儿肺炎,根据治疗方法不同分为观察组和对照组各39例。2组均给予对症治疗,在此基础上对照组给予阿莫西林-克拉维酸钾治疗,观察组给予枯草杆菌二联活菌颗粒联合阿莫西林-克拉维酸钾治疗。治疗7 d后比较2组疗效和康复指标(发热消失时间、咳嗽消失时间、肺部湿啰音消失时间、胸腔炎症吸收时间),治疗前后炎性指标[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞计数(WBC)]和免疫功能指标(CD3+、CD4+、CD8+、CD4+/CD8+),以及不良反应发生情况。结果2组总有效率比较差异无统计学意义(P>0.05)。观察组发热消失时间、咳嗽消失时间、肺部湿啰音消失时间、胸腔炎症吸收时间短于对照组(P<0.01)。治疗后,2组CRP、IL-6、TNF-α、WBC均降低,且观察组低于对照组(P<0.05,P<0.01)。治疗后,观察组CD3+、CD4+、CD4+/CD8+升高,CD8+降低,且CD3+、CD4+、CD4+/CD8+高于对照组,CD8+低于对照组(P<0.05,P<0.01)。观察组不良反应发生率低于对照组(P<0.05)。结论枯草杆菌二联活菌颗粒联合阿莫西林-克拉维酸钾治疗新生儿肺炎,可加快患儿症状体征缓解,缩短康复进程,增强抗感染作用,改善细胞免疫功能,减少腹泻不良反应的发生。 Objective To investigate the efficacy of Live Combined Bacillus Subtilis and Enterococcus Faecium Granules With Multivitamines combined with Amoxicillin-clavulanate Potassium in the treatment of neonatal pneumonia and its effect on the recovery process and inflammatory indexes.Methods A total of 78 patients with neonatal pneumonia admitted to the hospital from December 2020 to May 2022 were selected and divided into observation group(n=39)and control group(n=39)according to different treatment methods.Both groups were given symptomatic treatment.On this basis,the control group was given Amoxicillin-clavulanate Potassium treatment,and the observation group was given Live Combined Bacillus Subtilis and Enterococcus Faecium Granules With Multivitamines combined with Amoxicillin-clavulanate Potassium treatment.At 7 days after treatment,the curative effect and rehabilitation indicators(time to disappearance of fever,disappearance of cough and disappearance of pulmonary rales,and absorption time of pleural effusion),inflammatory indexes[C reactive protein(CRP),interleukin and interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),white blood cell count(WBC)]before and after treatment,immune function indicators(CD3+,CD4+,CD8+,CD4+/CD8+),and the occurrence of adverse reactions were compared between the two groups.Results There was no significant difference in the total effective rate between the two groups(P>0.05).The time to disappearance of fever,disappearance of cough,disappearance of pulmonary rales,and absorption time of pleural effusion in the observation group were shorter than those in the control group(P<0.01).After treatment,CRP,IL-6,TNF-αand WBC in both groups were decreased,which were lower in the observation group than in the control group(P<0.05,P<0.01).After treatment,CD3+,CD4+,CD4+/CD8+in the observation group increased,while CD8+decreased;CD3+,CD4+,CD4+/CD8+were higher than those in the control group,and CD8+was lower than that in the control group(P<0.05,P<0.01).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion Live Combined Bacillus Subtilis and Enterococcus Faecium Granules With Multivitamines combined with Amoxicillin-clavulanate Potassium in the treatment of neonatal pneumonia can accelerate the relief of symptoms and signs,shorten the recovery process,enhance the anti-infection effect,improve cellular immune function,and reduce the incidence of adverse reactions including diarrhea.
