摘要
目的调查全科住院医师职业发展现况,完善全科住培体系。方法通过问卷调查法,收集2008年7月—2020年7月上海市某三甲医院39名全科住培结业住院医师职业发展情况,调查项目包括基本情况、科研成果、工作满意度等,并使用SPSS 23.0统计学软件完成数据分析。结果多数调查对象收入水平在10~15万元之间(79.5%)。工作满意度显示:满意16名(41.0%),较满意15名(38.5%),一般8名(20.5%)。结业住院医师年收入与工作满意度成正相关(r=0.332,P=0.039)。不同性别、学历、毕业院校、工作年限的全科住培结业住院医师收入及发表中文核心期刊文章数量差异均无统计学意义(P>0.05)。硕士学员毕业后参加科研工作的比例(50.0%)高于本科学员(5.7%),差异有统计学意义(P=0.045)。不同学历、毕业院校、工作年限组的文章发表差异无统计学意义(P>0.05)。结论用人单位首先应考虑从收入出发,提升全科医师就业满意度。同时考虑优化分配机制,体现差异化,提高激励效果。就业后住院医师科研成果积累缓慢,存在一定科研懈怠,985/211院校全科医师并没有表现出较高科研水平。用人单位应重视全科住院医师科研发展,加强督促引导,搭建良好的科研平台。
Objective To investigate the career development status of general practitioners,and improve the general practice residency training system.Methods Through the questionnaire survey method,the professional development status of 39 general practitioners who completed the training in a tertiary hospital in Shanghai from July 2008 to July 2020 was collected.The survey items included basic information,scientific research results,job satisfaction,etc,and used SPSS 23.0 software to complete data analysis.Results The income level of most survey respondents was between 100,000 and 150,000 yuan(79.5%).Job satisfaction showed that:16 people were satisfied(41%),15 people were more satisfied(38.5%),and 8 people(20.5%)were generally satisfied.The annual income of graduated residents was positively correlated with job satisfaction(r=0.332,P=0.039).There was no statistically significant difference in the income of general practitioners who had completed residency training in different genders,educational backgrounds,graduate schools,and working years(P>0.05),so was the number of articles published in core journals Chinese.The average number of Chinese core journals published by female graduate residents was higher than that of male graduate residents,and the difference was statistically significant(P=0.006).The proportion of postgraduate students participating in scientific research after graduation was 50.0%higher than that of undergraduate students 5.7%,and the difference was statistically significant(P=0.045).There was no statistically significant difference in the publication of articles between groups of different educational backgrounds,graduate schools,and working years(P>0.05).Conclusion Employers should first consider increasing the satisfaction of general practitioners based on income.At the same time,consider optimizing the distribution mechanism,reflect differentiation,and improve the incentive effect.Residents have been slowly accumulating scientific research results after employment,and there is a certain degree of scientific research slack.General practitioners of 985/211 colleges and universities have not shown a high level of scientific research.Employers should attach importance to the research development of general resident physicians,strengthen supervision and guidance,and build a good scientific research platform.
