布地奈德联合沙丁胺醇雾化吸入对支气管肺炎患儿的肺功能、血气指标的影响

Effects of Budesonide Combined with Salbutamol Atomization Inhalation on Pulmonary Function and Blood Gas Indexes in Children with Bronchopneumonia

目的:探讨布地奈德联合沙丁胺醇雾化吸入对支气管肺炎患儿的肺功能、血气指标的影响。方法:回顾性分析2019年2月至2021年5月上饶市立医院支气管肺炎患儿82例,依据治疗方式划分成联合组(n=41)和常规组(n=41)。常规组选取传统治疗,联合组在常规组的基础上加用布地奈德联合沙丁胺醇。对比两组疗效,症状改善时间,肺功能[肺活量(FVC)、呼气峰值速度(PEF)、第一秒呼气容积(FEV1)],血气分析[氧饱和(SaO_(2))、氧化碳分压(PaCO_(2))],炎症反应[白细胞计数(WBC)、正常C反应蛋白(CRP)、白细胞介素4(IL-4)],不良反应变化。结果:联合组总有效率95.12%(39/41)比常规组75.61%(31/41)高(P<0.05);联合组住院时长、气喘消失时长、啰音消失时长、止咳时长、退热时长均短于常规组(P<0.05);联合组FEV1、FVC、PEF数值均高于常规组(P<0.05)。治疗后联合组SaO_(2)含量高于常规组,且PaCO_(2)含量低于常规组(P<0.05)。联合组CRP、WBC、IL-4含量低于常规组(P<0.05)。联合组不良反应12.20%(5/41)与常规组19.51%(8/41)对比无明显变化(P>0.05)。结论:布地奈德结合沙丁胺醇雾化吸入对于支气管肺炎患儿效果显著,可减轻炎性反应,缓解临床症状,改善血气水平和肺功能,且具有安全性,值得临床进一步推广。

Objective:To investigate the effect of budesonide combined with salbutamol atomization inhalation on pulmonary function and blood gas indexes in children with bronchopneumonia.Methods:82 children with bronchopneumonia in Shangrao Municipal Hospital from February 2019 to May 2021 were retrospectively analyzed.They were divided into combined group(n=41)and routine group(n=41)according to the treatment method.The traditional treatment was selected in the routine group,and budesonide combined with salbutamol was fundamentally selected in the combined group.The curative effect,symptom improvement time,lung function[vital capacity(FVC),peak expiratory velocity(PEF),first second expiratory volume(FEV1)],blood gas analysis[oxygen saturation(SaO_(2)),partial pressure of oxidized carbon(PaCO_(2))],inflammatory reaction[leukocyte count(WBC),normal C-reactive protein(CRP),interleukin-4(IL-4)],and adverse reaction changes were compared between the two groups.Results:The total effective rate of the combined group was 95.12%(39/41)higher than that of the conventional group 75.61%(31/41)(P<0.05);the length of hospitalization,the disappearance of asthma,the disappearance of rales,cough and fever in the combined group were shorter than those in the conventional group(P<0.05);FEV1,FVC and PEF in the combined group were higher than those in the conventional group(P<0.05);the content of SaO_(2) in the combined group was higher than that in the conventional group,and the content of PaCO_(2) was lower than that in the conventional group(P<0.05);the contents of CRP,WBC and IL-4 in the combined group were lower than those in the routine group(P<0.05);the adverse reactions in the combined group were 12.20%(5/41)and no significant changes compared with 19.51%(8/41)in the conventional group(P>0.05).Conclusion:Budesonide combined with salbutamol atomization inhalation has a significant effect on children with bronchopneumonia,which can reduce inflammatory reactions,alleviate clinical symptoms,improve blood gas levels and lung function,and has safety,which is worthy of further clinical promotion.
