阿托伐他汀联合替格瑞洛治疗不稳定型心绞痛的效果和安全性

Efficacy and safety of atorvastatin combined with ticagrelor in the treatment of unstable angina pectoris

目的分析阿托伐他汀联合替格瑞洛治疗不稳定型心绞痛(UAP)的效果和安全性。方法抽取2017年10月至2020年9月解放军联勤保障部队第九九〇医院(信阳院区)收治的UAP患者90例,按照随机数字表法分为替格瑞洛组和氯吡格雷组,每组45例。氯吡格雷组予以阿托伐他汀联合氯吡格雷治疗,替格瑞洛组予以阿托伐他汀联合替格瑞洛治疗。比较两组疗效、心绞痛发生情况、血清因子[氨基末端脑钠肽前体(NT-proBNP)、基质金属蛋白酶-9(MMP-9)]水平、血脂[总胆固醇(TC)、三酰甘油(TG)]水平及不良反应发生情况。结果替格瑞洛组总有效率(95.56%,43/45)高于氯吡格雷组(73.33%,33/45),P<0.05。治疗后,替格瑞洛组心绞痛发作次数少于氯吡格雷组,发作时间短于氯吡格雷组(P<0.05)。治疗后,替格瑞洛组NT-proBNP、MMP-9、TC、TG水平均低于氯吡格雷组(P均<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论采用阿托伐他汀联合替格瑞洛治疗UAP能提升治疗效果,对缓解机体炎症具有良好效果,并能促进血脂改善,减少心绞痛发作。

Objective To analyze the efficacy and safety of atorvastatin combined with ticagrelor in the treatment of unstable angina pectoris(UAP).Methods A total of 90 patients with UAP treated in Xinyang Hospital of No.990 Hospital of the Joint Logistic Support Force from October 2017 to September 2020 were selected.The patients were divided into ticagrelor group and clopidogrel group by random number table method,with 45 cases in each group.The clopidogrel group was treated with atorvastatin combined with clopidogrel,and the ticagrelor group was treated with atorvastatin combined with ticagrelor.The curative effect,occurrence of angina pectoris,serum factors including N-terminal pro-brain natriuretic peptide(NT-proBNP)and matrix metalloproteinase 9(MMP-9),blood lipid assessed by total cholesterol(TC)and triglyceride(TG),and incidence of adverse reactions were compared between the two groups.Results The total effective rate in the ticagrelor group(95.56%,43/45)was higher than that in clopidogrel group(73.33%,33/45),P<0.05.After treatment,ticagrelor group had fewer angina attacks and shorter attack time than copidogrel group(P<0.05).After treatment,levels of NT-probNP,MMP-9,TC and TG in the ticagrelor group were lower than those in the clopidogrel group(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Atorvastatin combined with ticagrelor in the treatment of UAP can improve the therapeutic effect,relieve inflammation,improve blood lipids,and reduce angina pectoris.
