摘要
目的探讨小规模限制性在线课程(small private online course,SPOC)辅助的基于案例学习(case-based learning,CBL)-翻转课堂混合教学模式在伤口清创缝合教学中的应用效果。方法采用便利抽样方法,选取2020年6月至2021年7月期间,在浙江大学医学院附属第二医院整形科进行住院医师规范化培训轮转的58名住院医师为研究对象,将双号月份轮转的28名住院医师设为试验组,单号月份轮转的30名住院医师设为对照组。试验组住院医师采用SPOC辅助的CBL-翻转课堂混合教学模式,对照组住院医师采用传统技能培训模式。伤口清创缝合教学结束后,通过理论知识、专科技能操作和临床思维考核成绩和教学满意度问卷调查结果,比较两组住院医师培训效果。结果试验组住院医师在理论知识、专科技能操作考核中的成绩均显著高于对照组住院医师,其中理论知识考核成绩差异具有统计学意义[(84.46±7.04)分vs.(74.83±16.26)分,t=2.890,P<0.05],专科技能考核成绩差异亦具有统计学意义[(88.64±2.02)分vs.(85.10±5.31)分,U=271.500,P<0.05];两组住院医师临床思维考核成绩比较,其差异无统计学意义[(89.29±2.92)分vs.(89.03±3.29)分,U=389.000,P>0.05]。在教学满意度调查问卷中,除了提高知识理解能力之外,试验组住院医师各项内容评分均显著高于对照组住院医师,其差异均具有统计学意义(均P<0.05)。结论SPOC辅助的CBL-翻转课堂混合教学模式有利于改善住院医师伤口清创缝合技能操作教学效果,提高教学满意度。
Objective To observe and explore the application effect of CBL(case-based learning)-flip classroom mixed teaching mode assisted by SPOC(small private online course)in wound debridement and suture teaching.Methods From June 2020 to July 2021,58 residents who underwent resident standardized training rotation in the Plastic Surgery Department of the Second Affiliated Hospital Zhejiang University School of Medicine were selected by convenience sampling as the research object.The rotation doctors in double month were included in the experimental group(n=28 people),while those in single month were included in the control group(n=30 people).On the basis of selecting CBL teaching model,the experimental group realized flipped classroom teaching with the help of SPOC,while the control group used the mode of traditional skill training.After teaching,the teaching effect was evaluated and compared between two groups by means of graduation theory,operation and clinical thinking assessment,teaching satisfaction questionnaire and so on.Results The results of theory and operation examination in the experimental group were significantly higher than those in the control group at the end of the month(84.46±7.04 vs.74.83±16.26,t=2.890,P<0.05;88.64±2.02 vs.85.10±5.31,U=271.500,P<0.05),but there was no significant difference in the clinical thinking examination between the two groups(89.29±2.92 vs.89.03±3.29,U=389.000,P>0.05).The evaluation results of four aspects of the teaching satisfaction questionnaire in the experimental group were significantly higher than those in the control group(all P<0.05).Conclusions The CBL-flipped classroom mixed teaching mode assisted by SPOC is helpful to improve the training effect of the debridement and suture of surgical wounds,and increase the satisfaction of operation teaching.
