摘要
目的探讨人免疫球蛋白治疗新生儿感染性肺炎的价值及对患儿血气分析指标的影响.方法选取天津市静海区医院2018年8月-2020年8月期间新生儿感染性肺炎患儿48例,采用随机数字表法将其分为观察组与对照组各24例,给予所有患儿常规治疗,包括抗感染、补液、纠正酸碱失衡、纠正电解质紊乱,保持温度、湿度适宜等;当患儿出现呼吸困难的症状时,注意保持呼吸道畅通,及时供氧.观察组在上述基础上给予静注人免疫球蛋白400mg/kg,每日1次.两组持续治疗5d.观察两组患儿临床疗效、发热、咳嗽、啰音、哮鸣音、住院时间及血气分析结果有无差异.结果观察组总有效率95.8%(23/24)高于对照组的83.33%(20/24),且有统计学差异(P<0.05).观察组的发热、咳嗽、啰音、哮鸣音、住院时间均短于对照组(P<0.05);治疗后观察组的PaO2高于对照组,SaO2高于对照组(P<0.05);结论人免疫球蛋白应用于新生儿感染性肺炎的治疗中,可提高临床疗效,促进患儿的症状体征改善,可明显改善血气分析指标,安全性良好.
Objective To investigate the value of human immunoglobulin in the treaetnmt of neonatal infectious pneumonia and its effec t on blood gas analysis indexes.Methods 48 children with neonatal infectious pneumonia in Tianjin Jinhgai district hospital from August 2018 to August 2020 were randomly divided into observation grpo uand control group.All children were given routine treatment,nicluding anti infection,rehydration,correction of acid-base imbalance,correction of electrolyte disorder,maintaining appropriate temperature and humidity,etc;when children have symptoms of dyspnea,pay attentoin to keep the respiratory tract unblocked and provide oxygenn itime.On the basis of the above,the observation group was given intravoeuns injection of human immunoglobulin 400mg/kg once a day.The wot groups were treated for 5 days.The clinical efficacy,fever,cough,ralew,heeze,hospitalization time and blood gas analysis results of het two groups were observed.Results The total effective rate in the observation group was 95.8%(/2234)higher than 83.33%(20/24)in the contro l group,and there was significant difference P(<0.05).The fever,cough,rale,wheeze and hospital stay in the obesrvation group were shorter than those in the control group P(<0.05);after treatment,PaO2 and SaO2 in the observation group ewre higher than those in the contro l group(P<0.05).Conclusion The application of human immunoglobulin in the treatment of onneatal infectious pneumonia can improve the clinical efficacy,improve the symptoms and signs of children,sginificantly improve the blood gas analysis indexes,and has goo dsafety.
