自拟痹痛散外敷联合红外线局部照射治疗脑卒中后肩手综合征的临床疗效观察

摘要 目的观察并研究自拟痹痛散外敷联合红外线局部照射治疗脑卒中后肩手综合征的临床疗效.方法选取2020年4月-2022年4月期间于台山市中医院脑病科门诊就诊或住院的70例脑卒中后肩手综合征患者作为研究对象,采取随机数字表法将70例入选对象分别纳入对照组(n=35)及治疗组(n=35).给予对照组关节肌力、耐力训练等西医常规康复治疗,治疗组在对照组基础上采用自拟痹痛散外敷联合红外线局部照射治疗.对比两组疗效以及治疗前后的证候积分(肩手痛、皮肤潮红、手指屈曲受限、手部肿胀),参考视觉模拟评分法(VAS)及上肢简化Fugl-Meyer评分(FMA)量表评估两组关节疼痛及肩手活动功能改善情况.结果治疗组治疗总有效率为97.14%,高于对照组的77.14%(P<0.05);与对照组相比,治疗组治疗后的肩手痛、皮肤潮红等证候积分较低,VAS评分较低,FMA评分较高(P<0.05).结论对脑卒中后肩手综合征患者采取自拟痹痛散外敷联合红外线局部照射治疗可取得显著疗效,显著改善各相关症状并减轻关节处疼痛,同时也改善肩手活动功能. Objective To observe and study the clinical efficacy of external application of self-made Bitong powder combined with local infrared irradiation in the treatment of post-stroke shoulder-hand syndrome.Methods A total of 70 patients with post-stroke shoulder-hand syndrome who were treated or hospitalized in the Department of Encephalopathy,Taishan Hospital of Traditional Chinese Medicine from April 2020 to April 2022 were selected as the research subjects,and the 70 selected subjects were included in the random number table method.Control group(n=35)and treatment group(n=35).The control group was given routine rehabilitation therapy of western medicine such as joint muscle strength and endurance training,and the treatment group was given external application of self-made Bitong powder combined with infrared local irradiation on the basis of the control group.The curative effect and symptom scores(shoulder hand pain,skin flushing,limited finger flexion,hand swelling)before and after treatment were compared between the two groups,and the joints of the two groups were evaluated with reference to the visual analogue scale(VAS)and the simplified Fugl-Meyer score(FMA).Improvement in pain and shoulder-hand mobility.Results The total effective rate of the treatment group was 97.14%,which was higher than 77.14%of the control group(P<0.05).And the FMA score was higher(P<0.05).Conc lusion The external application of self-made Bitong powder combined with local infrared irradiation can achieve significant curative effect in patients with shoulder-hand syndrome after stroke,which can improve the related symptoms,relieve joint pain,and improve shoulder-hand mobility.
