超声多参数判断甲状腺微小乳头状癌颈部淋巴结转移的相关性研究

摘要 目的:探讨超声多参数判断甲状腺微小乳头状癌颈部淋巴结转移的相关性研究。方法:回顾性分析2014年1月—2017年12月在齐齐哈尔医学院临床病理诊断中心治疗的379例甲状腺微小乳头状癌(papillary thyroid microcarcinoma,PTMC)患者的临床资料,根据是否发生颈部淋巴结转移(cervical lymph node metastasis,CLNM)进行分组,观察组(189例)已发生CLNM,对照组(190例)无CLNM,对比两组的一般资料及解剖学特征、超声特征,比较患者特征与颈部淋巴结转移的情况,进行单因素和多因素分析。结果:两组年龄、性别、桥本背景、合并结节性甲状腺肿、肿瘤位置、与包膜关系等相比均无显著差异(P>0.05),而两组侧别比例存在显著差异(P<0.05);观察组在肿瘤病灶数目、形态、边界、钙化灶、肿瘤直径等超声特征检出情况与对照组存在显著差异(P<0.05),但两组纵横比、声晕、回声、后方回声、血流等超声特征无显著差异(P>0.05);多因素Logistic回归分析显示,肿瘤直径>7 mm、双侧病灶、边界不清是PTMC患者CLNM的独立危险因素(P<0.05)。结论:超声多参数判断PTMC患者CLNM有较高价值,能分析不同特征PTMC的超声特征,明确PTMC颈部淋巴结转移的高危因素,为临床早期诊治提供可靠依据,利用患者早期进行治疗。
