全髋关节置换术与半髋关节置换术对股骨颈骨折的疗效分析

Efficacy Analysis of Total Hip Arthroplasty and Hemiarthroplasty for Femoral Neck Fracture

目的:分析股骨颈骨折患者应用全髋关节置换术和半髋关节置换术的近期疗效。方法:选取2019年1月-2021年12月海门区第三人民医院收治的60例股骨颈骨折患者为研究对象,根据手术方法分为研究组和对照组,各30例。研究组实施半髋关节置换术,对照组实施全髋关节置换术。比较两组近期疗效、并发症发生率以及治疗前后的髋关节功能评分。结果:研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05);研究组并发症发生率低于对照组,差异有统计学意义(P<0.05);治疗前,两组髋关节功能评分比较,差异无统计学意义(P>0.05);治疗后,两组髋关节评分均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05)。结论:半髋关节置换术的近期疗效更好,患者术后近期髋关节功能更好,全髋关节置换术远期疗效好但并发症发生率较高,二者各有优缺点,临床应根据患者实际情况进行选择。

Objective:To analyze the short-term efficacy of total hip arthroplasty and hemiarthroplasty in patients with femoral neck fracture.Methods:A total of 60 patients with femoral neck fractures who were treated in the Third People's Hospital of Haimen District from January 2019 to December 2021 were selected as the study subjects.According to the surgery methods,the patients were divided into study group and control group,with 30 cases in each group.The study group underwent hemiarthroplasty and the control group underwent total hip arthroplasty.The short-term efficacy,complication rate and hip joint function score before and after treatment were compared between two groups.Results:The total effective rate of treatment in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).The complication rate in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in hip joint function scores between the two groups(P>0.05).After treatment,hip joint function scores in two groups were higher than those before treatment,and the study group was higher than the control group,with a statistically significant difference(P<0.05).Conclusion:The hemiarthroplasty has better short-term efficacy and short-term postoperative hip joint function,while total hip arthroplasty has good long-term efficacy but high complication rates.Both treatments have their own advantages and disadvantages and may be selected clinically according to patient's actual condition.
