胸腔镜下微创肺段切除术对肺部磨玻璃结节患者疗效及肺功能的影响

The effect of thoracoscopic minimally invasive segmentectomy on the curative effect and pulmonary function of patients with pulmonary ground glass nodules

目的探讨胸腔镜下微创肺段切除术对肺部磨玻璃结节患者疗效及肺功能的影响。方法选取2019年1月~2021年12月收治的60例肺部磨玻璃结节患者,随机数字表法分为试验组(胸腔镜肺段切除术,30例)和对照组(胸腔镜肺叶切除术,30例)。评价两组疗效,观察两组基本手术指标,检测两组手术前后肺功能,统计并发症。结果两组手术成功率均为100%。试验组手术时间(136.51±17.84)min、引流时间(2.84±0.65)d、术后住院时间(6.48±1.33)d短于对照组,术中出血量(34.60±10.11)ml少于对照组,差异有统计学意义(P<0.05)。试验组术后FVC(2.72±0.17)L、MVV(92.53±6.80)L/min、FEV1(84.19±2.86)%高于对照组,RV(1.31±0.15)L低于对照组,差异有统计学意义(P<0.05)。试验组并发症发生率(3.33%)与对照组(6.67%)差异无统计学意义(P>0.05)。结论胸腔镜肺段切除术治疗肺部磨玻璃结节可以获得与胸腔镜肺叶切除术相当的疗效与安全性,但前者手术创伤小,术中出血少,术后恢复快,对肺功能影响小。

Objective To investigate the effect of thoracoscopic minimally invasive segmentectomy on the curative effect and pulmonary function of patients with pulmonary ground glass nodules.Methods A total of 60 patients with pulmonary ground glass nodules who were treated in our hospital from January 2019 to December 2021 were selected and randomly divided into the experimental group(thoracoscopy segmentectomy,30 cases)and the control group(thoracoscopy lobectomy surgery,30 cases).The curative effect of the two groups was evaluated,the basic surgical indexes of the two groups were observed,the lung function before and after the operation was detected,and the complications were counted.Results The surgical success rate in both groups was 100%.The operation time(136.51±17.84)min,drainage time(2.84±0.65)d and postoperative hospital stay(6.48±1.33)d in the experimental group were shorter than those in the control group,and the intraoperative blood loss(34.60±10.11)ml was less than that in the control group.The difference was statistically significant(P<0.05).The postoperative FVC(2.72±0.17)L,MVV(92.53±6.80)L/min,and FEV1(84.19±2.86)%of the experimental group were higher than those of the control group,and the RV(1.31±0.15)L was lower than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications between the experimental group(3.33%)and the control group(6.67%)(P>0.05).Conclusion Thoracoscopic segmentectomy for the treatment of pulmonary ground glass nodules can achieve the same efficacy and safety as thoracoscopic lobectomy,but the former has less surgical trauma,less intraoperative blood loss,faster postoperative recovery,and less impact on lung function.
