超声新技术在甲状腺乳头状癌颈部淋巴结转移中的应用进展

摘要 甲状腺乳头状癌(papillary thyroid carcinoma,PTC)是甲状腺癌最常见的病理类型,近些年其发病率呈明显上升趋势。PTC的颈部淋巴结转移较为常见,是影响患者预后的重要因素之一。因此,术前对颈部淋巴结转移情况进行正确评估对手术方案选择及预后评估都有重要影响。近年来,高频超声、超声造影、弹性成像、超微血流成像、介入超声、细针穿刺洗脱液甲状腺球蛋白(fine needle aspiration thyroglobulin,FNA-Tg)测定等超声新技术以其简便、无辐射、高效等优势广泛应用于临床,因此,相比较其他影像学检查,超声可作为PTC颈部转移淋巴结(metastatic lymph nodes,MLN)情况评估的首选检查方法。本文对近年来超声检查新技术评估PTC颈部MLN的相关文献进行复习,以期为临床的诊断应用提供一定参考。 Papillary thyroid carcinoma(PTC)is the most common pathologic type of thyroid cancer,and its incidence has increased significantly in recent years,and it is more common in women than men.Lymph node metastasis of PTC is common and is one of the important factors affecting the prognosis of patients.Therefore,correct preoperative evaluation of cervical lymph node metastasis has an important impact on the selection of surgical plan and prognosis evaluation.In recent years,the high frequency ultrasound,ultrasound imaging,elasticity imaging,amd blood flow imaging,interventional ultrasound,and fine needle aspiration thyroglobulin(FNA-Tg)measurement such as ultrasonic technology,with its simple,no radiation,high efficiency advantages,were widely used in clinical.Therefore,compared with other imaging examination,ultrasound techniques can be used as the first choice for the PTC metastasis lymph nodes(MLN)situation assessment.In this paper,we reviewed the literatures related to the evaluation of PTC metastatic lymph nodes with new ultrasound techniques in recent years,in order to provide some reference for clinical diagnosis and application.
