江苏省居家养老老年人健康管理系统指标体系构建

Construction of the index system of the health management system for the elderly at home in Jiangsu province

文章在分析我国居家养老现状、国家政策支持以及现代社会老年人及其子女对于养老的需求转变基础上,得出构建居家养老老年人健康管理系统的必要性。通过分析、借鉴国内外居家老年人健康管理系统相关案例,如美国家庭智慧健康系统、美国联合健康集团、美国在线健康管理系统、加拿大健康管理中心、日本健康管理平台以及国内的蓝熙、养老云智慧平台、社村通智慧居家养老服务平台、江苏康优凯欣智慧养老平台,对比得出国内外健康管理的不同。综合江苏省具体情况,从基本健康档案、健康风险评估、慢性病管理这3个维度提出构建符合江苏省居家养老老年人健康管理需求的健康指标体系。

Based on the analysis of the current situation of home-based elderly care in China,the national policy support and the transformation of the needs of the elderly and their children in modern society for elderly care,the paper concludes that it is necessary to build a health management system for home-based elderly care.Through analysis and reference to relevant cases of home-based elderly health management systems at home and abroad,such as the American Family Intelligent Health System,the American United Health Group,the American Online Health Management System,the Canada Wellness Institute,the Japanese Health Management Platform,as well as the domestic Lanxi,the Elderly Care Cloud Intelligent Platform,the Shecuntong Smart Home based Elderly Care Service Platform,and the Jiangsu COMMUNICATION Smart Elderly Care Platform,compare the differences of health management at home and abroad.Based on the specific situation of Jiangsu province,it is proposed to build a health indicator system that meets the health management needs of the elderly at home in Jiangsu province from the three dimensions of basic health records,health risk assessment and chronic disease management.
