帕金森病病人社会参与现状及影响因素研究

Analysis of status quo and influencing factors of social participation of patients with Parkinson′s disease

目的:调查帕金森病病人社会参与水平现状,分析其影响因素。方法:选取2021年5月—2021年10月在太原市2所三级甲等医院门诊就诊的帕金森病病人239例作为研究对象。采用一般资料问卷、中文版自主参与问卷(IPA)、改良的Hoehn⁃Yahr分级量表、医院焦虑及抑郁自评量表(HADS)、社会支持量表(SSRS)、慢性病病耻感量表(SSCI)进行调查。结果:帕金森病病人IPA总分为(48.98±16.92)分,各维度中,家庭角色自主参与维度均分[(2.09±0.88)分]最高,其次为室内自主参与维度[(2.09±0.87)分]和室外自主参与维度[(2.05±0.75)分],社会生活和社会关系维度均分[(1.58±0.62)分]最低。多元线性回归分析结果显示,年龄、Hoehn⁃Yahr分级、HADS中的抑郁情绪得分、SSRS中的对支持的利用度得分、SSCI中的内在病耻感得分5个变量进入模型(P<0.05)。结论:帕金森病病人社会参与水平欠佳,医务人员应关注其影响因素,制定并实施相应的干预措施,提高帕金森病病人社会参与水平。

Objective:To investigate status quo of social participation of patients with Parkinson′s disease,and analyze its influencing factors.Methods:239 patients with Parkinson′s disease in outpatient clinics of two tertiary grade A hospitals in Taiyuan from May 2021 to October 2021 were selected as study subjects.General information questionnaire,Impact on Participation and Autonomy Questionaire(IPA),Modified Hoehn⁃Yahr Rating Scale,Hospital Anxiety and Depression Scale(HADS),Social Support Rating Scale(SSRS)and stigma scale for chronic illness(SSCI)were used to investigate.Results:Total IPA score of Parkinson′s patients was 48.98±16.92.Among all dimensions,the average score of family role independent participation 2.09±0.88 was the highest,followed by indoor independent participation 2.09±0.87 and outdoor independent participation 2.05±0.75,and the average score of social life and social relations 1.58±0.62 was the lowest.The results of multiple linear regression analysis showed that age,Hoehn⁃Yahr grade,depressive mood score in HADS,utilization of support score in SSRS and intrinsic shame score in SSCI entered the model(P<0.05).Conclusions:Level of social participation of patients with Parkinson′s disease is not good.Medical stuff should pay attention to its influencing factors,formulate and implement corresponding intervention measures to improve the level of social participation of patients with Parkinson′s disease.
