摘要
目的探讨24 h动态心电图在心房颤动(Af)伴长R-R间期检查中的应用效果。方法回顾性分析2019年3月至2022年3月连云港市第二人民医院收治的Af伴长R-R间期患者500例的临床资料,所有患者均行24 h动态心电图检查,对患者Af伴长R-R间期发生时间进行统计,根据动态心电图检测结果分为睡眠相关组(在21:00~6:00出现长R-R间期,319例)与非睡眠相关组(非睡眠期间出现长R-R间期,181例),比较两组患者的心电图特征及逸搏心律、逸搏及昏厥发生情况。结果两组患者的性别、年龄、NYHA分级、疾病类型及Af持续时间比较,差异无统计学意义(P>0.05);睡眠相关组最慢心室率、平均心室率及最快心室率均高于非睡眠相关组,差异有统计学意义(P<0.05);睡眠相关组的逸搏心律、逸搏及昏厥发生率均低于非睡眠相关组,差异有统计学意义(P<0.05)。结论24 h动态心电图可有效检测Af伴长R-R间期的特征,Af伴长R-R间期多发生于睡眠状态,应密切关注Af伴长R-R间期患者的24 h心电图检查情况,及时采取干预措施。
Objective To investigate the characteristics of 24-hour ambulatory electrocardiogram in atrial fibrillation(Af)with long R-R interval.Methods The clinical data of 500 patients with Af with long R-R interval treated in Lianyungang Second People's Hospital from March 2020 to March 2022 were analyzed retrospectively.All patients underwent 24-hour ambulatory ECG.The occurrence time of Af with long R-R interval was counted.According to the occurrence time,they were divided into sleep related group(long R-R interval occurred from 21:00 to 6:00,319 cases)and non sleep related group(long R-R interval occurred during non sleep,181 cases).The ECG characteristics,escape rhythm Escape stroke and syncope.Results There were no significant differences in gender,age,NYHA grade,disease type and Af duration between sleep related group and non sleep related group(P>0.05).The slowest ventricular rate,mean ventricular rate and fastest ventricular rate in sleep related group were higher than those in non sleep related group,with statistically significant differences(P<0.05).The incidences of escape rhythm,escape and syncope in sleep related group were lower than those in non sleep related group,with statistically significant differences(P<0.05).Conclusion 24 h ambulatory electrocardiogram can effectively detect the characteristics of Af with long R-R interval.Af with long R-R interval often occurs in sleep.We should pay close attention to the 24 h ECG examination of patients with Af with long R-R interval and take intervention measures in time.
