Rotterdam CT评分与Helsinki CT评分对脑损伤患者预后的评估作用对比

Comparison of Rotterdam CT score and Helsinki CT score in evaluating the prognosis of patients with brain injury

目的比较Rotterdam CT评分、Helsinki CT评分对脑损伤患者预后的评估价值。方法回顾性分析2019年1月至2021年3月江西省德兴市人民医院收治的70例脑损伤患者的临床资料,所有患者均在入院72 h内或术前完成颅脑CT检查,所有患者的CT影像均进行Rotterdam CT评分、Helsinki CT评分,并随访至2022年3月,采用格拉斯哥预后评分(GOS)评估患者的预后情况,分析Rotterdam CT评分、Helsinki CT评分对脑损伤预后的评估价值。结果70例脑损伤患者中预后良好52例,占比74.29%,预后不良18例,占比25.71%。预后良好患者的Rotterdam CT评分、Helsinki CT评分低于预后不良患者,差异有统计学意义(P<0.05)。绘制受试者工作特征(ROC)曲线,结果显示,Rotterdam CT评分、Helsinki CT评分对脑损伤患者预后评估的曲线下面积(AUC)均>0.7,且Rotterdam CT评分与Helsinki CT评分的AUC比较,差异无统计学意义(Z=0.238,P=0.812)。结论Rotterdam CT评分、Helsinki CT评分对脑损伤患者预后均有一定的评估价值,可为临床预后评估提供客观的参考。

Objective To compare the prognostic value of Rotterdam CT score and Helsinki CT score in patients with brain injury.Methods Clinical data of 70 patients with brain injury admitted in Dexing People's Hospital from January 2019 to March 2021 were retrospectively analyzed.All patients underwent craniocerebral CT examination within 72 h of admission or before surgery.CT images of all patients were evaluated by Rotterdam CT score and Helsinki CT score.The patients were followed up to March 2022.The outcomes of patients were evaluated by The Glasgow outcome scale(GOS),and the evaluation value of Rotterdam CT score and Helsinki CT score for the prognosis of brain injury were analyzed.Results Among 70 patients with brain injury,52 had good prognosis,accounting for 74.29%,and 18 had poor prognosis,accounting for 25.71%.Rotterdam CT score and Helsinki CT score of patients with good prognosis were lower than those of patients with poor prognosis,and the differences were statistically significant(P<0.05).The receiver operating characteristic(ROC)curve was drawn.The results showed that the area under the curve(AUC)of Rotterdam CT score and Helsinki CT score for the prognosis assessment of patients with brain injury were>0.7,and there was no statistically significant difference in the AUC of Rotterdam CT score and Helsinki CT score(Z=0.238,P=0.812).Conclusion Rotterdam CT score and Helsinki CT score have certain evaluation value for the prognosis of patients with brain injury,and can provide objective reference for clinical prognosis evaluation.
