摘要
目的研究B型胸腺瘤胸腺组织白细胞介素(IL)-1、IL-6及EBER-1基因表达与EB病毒(EBV)感染的关系。方法选择海南医学院第一附属医院心胸外科2016年2月-2021年4月收治的90例B型胸腺瘤患者作为研究对象,另选择同期因其他原因手术切除胸腺患者作为对照组(71例),进行回顾性分析,均检测患者胸腺组织IL-1、IL-6及EBER-1基因表达情况。分析上述因子在B型胸腺瘤各亚型以及临床分期中的分布情况,根据患者有无EBV感染分为感染组(49例)和未感染组(41例),通过Logistic回归分析IL-1、IL-6表达与EBV感染的关系。结果研究组患者IL-1、IL-6及EBER-1基因表达阳性率高于对照组(P<0.05);不同分期患者IL-1表达阳性率比较存在统计学差异,随着临床分期的升高,IL-1、EBER-1基因表达阳性率逐渐升高(P<0.05);Logistic回归分析显示临床分期(Ⅲ、Ⅳ期)、IL-1表达(阳性)和IL-6表达(阳性)是影响B型胸腺瘤患者EBV感染发生的影响因素(P<0.05)。结论IL-1、IL-6及EBER-1基因在B型胸腺瘤组织中广泛表达,且IL-1、EBER-1基因表达与胸腺瘤的临床分期相关,EBV感染与胸腺瘤的发生有一定的相关性,并可能通过引起患者体内IL-1、IL-10因子表达的改变促进疾病的进展。
OBJECTIVE To study the relationship between the expression of interleukin-1(IL-1),IL-6 and EBER-1 genes in type B thymoma tissue and Epstein-Barr virus(EBV)infection.METHODS Ninety patients with type B thymoma admitted to the cardiothoracic surgery department of the First Affiliated Hospital of Hainan Medical University from Feb 2016 to Apr 2021 were enrolled as the research subjects,and 71 patients who underwent thymus resection for other reasons were taken as the control group.A retrospective analysis was conducted in these patients and the expression of IL-1,IL-6 and EBER-1 genes in the thymus tissues were detected.The distribution of above factors in different subtypes and clinical stages of type B thymoma was analyzed.According to whether EBV infection occurred,the patients were divided into the infected group(49 cases)and uninfected group(41 cases).Multivariate Logistic regression analysis was performed to analyze the relationship between the expression levels of IL-1 and IL-6 and EBV infection.RESULTS The positive expression rates of IL-1,IL-6 and EBER-1 genes in the study group were significantly higher than those in the control group(P<0.05).There were significant differences in the positive expression rate of IL-1 among patients at different stages.The positive expression rates of IL-1 and EBER-1 genes increased with the increasing clinical stages(P<0.05).Multivariate Logistic regression analysis showed that clinical stages ofⅢandⅣ,positive expression of IL-1 and IL-6 were the risk factors for EBV infection in patients with type B thymoma(P<0.05).CONCLUSION IL-1,IL-6 and EBER-1 genes are widely expressed in type B thymoma tissues,and the expression of IL-1 and EBER-1 genes is related to the clinical stages of thymoma.EBV infection is related to the occurrence of thymoma,and promote the progression of the disease by causing variation levels of the expression of IL-1 and IL-10 in patients.
