营养支持对卡瑞利珠单抗联合化疗一线治疗晚期非小细胞肺癌疗效的影响

Effect of nutritional support on the efficacy of camrelizumab combined with chemotherapy in the first-line treatment of advanced non-small cell lung cancer

目的探讨营养支持对免疫联合化疗治疗晚期非小细胞肺癌患者疗效的影响。方法回顾性分析86例Ⅳ期非鳞非小细胞肺癌患者一线采用卡瑞利珠单抗联合培美曲塞+卡铂或顺铂方案治疗后的疗效及生活质量评估。参与营养干预者纳入干预组,未参与者为对照组。依据RECIST1.1标准进行肿瘤疗效评估,采用欧洲癌症研究及治疗组织生活质量核心量表及肺癌生活质量表评估营养支持对非小细胞肺癌患者生活质量的影响。结果干预组患者免疫联合化疗的客观缓解率、疾病控制率分别为61.4%、84.1%,明显高于对照组(客观缓解率38.1%、疾病控制率64.3%),差异具有统计学意义。干预组患者在躯体功能、认知功能、情绪功能、社会功能、角色功能领域中评分均高于对照组,疲劳、疼痛、恶心呕吐3个症状领域的生活质量评分均低于对照组,差异具有统计学意义(P<0.05)。结论营养干预在非小细胞肺癌患者免疫联合化疗中具有积极效果,显著改善患者的疗效和生活质量。

Objective To explore the effect of nutritional support on the efficacy of immunotherapy combined with chemotherapy in the first-line treatment of patients with advanced non-small cell lung cancer(NSCLC).Methods A retrospective analysis of 86 patients with stageⅣnon-squamous non-small cell lung cancer were retrospectively conducted.All patients were treated with camrelizumab combined with pemetrexed and carboplatin or cisplatin chemotherapy as the first-line treatment.Among them,patients who participated in nutrition intervention were included in the intervention group,while those who did not receive nutrition intervention were included in the control group.The tumor efficacy of NSCLC patients treated with immunotherapy combined with chemotherapy was evaluated according to the RECIST1.1 criteria.The European organization for research and treatment of cancer-quality of life questionnaire core 30(EORTC-QLQ-C30)and the quality of life questionnaire-lung cancer 13(QLQ-LC 13)were used to assess and compare the quality of life of patients.Results The objective remission rate(ORR)and disease control rate(DCR)of immunotherapy combined with chemotherapy in the intervention group(ORR:38.1%,DCR:64.3%)were higher than those in the control group(ORR:61.4%,DCR:84.1%),which had significantly statistical differences.The scores of physical function,cognitive function,emotional function,social function and role function in the intervention group were statistically higher than those in the control group,while the quality of life scores in the subgroups of fatigue,pain,nausea and vomiting were all statistically lower than those in the control group.Conclusion Nutritional intervention has positive effect in NSCLC patients who received immunotherapy combined with chemotherapy,which can significantly improve the tumor efficacy and quality of life of patients.
