摘要
目的本研究探讨垂体后叶素在治疗咯血时,对外周血钠离子水平的影响。方法共收集2016年1月至2021年12月因咯血住院治疗的患者124例,按使用药物的组合,分为垂体后叶素组、垂体后叶素联合酚妥拉明治疗组及对照组,分别对患者的临床资料进行回顾性分析。结果34例患者单纯使用垂体后叶素治疗,治疗前后血钠水平分别为(140.52±3.19)mmol/L,(133.99±6.34)mmol/L,治疗前后血钠水平变化具有显著统计学差异(t=5.639,P<0.001),38例联合组患者治疗前后血钠水平变化具有统计学差异(P<0.05),而52例对照组患者治疗前后血钠水平变化没有统计学差异(P>0.05)。垂体后叶素组低钠血症的发生率为50.00%,与联合组(23.68%)、对照组(9.63%)均有显著统计学差异。当泵入时间>48h及累计泵入量>50U时,低钠血症发生明显增加。结论垂体后叶素治疗咯血时可引起低钠血症,垂体后叶素使用超过48小时、累计使用量超过50U是发生低钠血症的风险因素,联合酚妥拉明治疗可降低低钠血症的发生。
Objective To investigate the effect of pituitrin on the level of serum sodium in treatment of hemoptysis.Methods A total of 124 patients hospitalized for hemoptysis from January 2016 to December 2021 were collected and divided into the pituitrin group,the pituitrin combined with phentolamine group and the control group according to the combination of drugs.Their clinical data were retrospectively analyzed.Results 34 patients were treated with pituitrin alone,and their blood sodium levels before and after treatment were(140.52±3.19)mmol/L and(133.99±6.34)mmol/L respectively(t=5.639,P<0.001).There were statistical differences in serum sodium levels of 38 patients in the combined group before and after treatment(P<0.05),while there was no significant statistical differences in serum sodium levels of 52 patients in the control group before and after treatment.The incidence of hyponatremia in the pituitrin group was 50.00%,which was significantly different from that in the combined group(23.68%)and the control group(9.63%).The incidence of hyponatremia increased significantly when the pumping time was more than 48h and the cumulative pumping volume was more than 50U.Conclusion Pituitrin can cause hyponatremia in treatment of hemoptysis.The use of pituitrin for more than 48 hours and the cumulative use of pituitrin for more than 50U are risk factors for the occurrence of hyponatremia.The combined treatment with phentolamine can reduce the occurrence of hyponatremia.
