摘要
目的探究新冠疫情下翻转课堂结合CBL在妇产科临床实习教学中的教学效果,为提高后疫情时代妇产科临床实习教学效率和质量总结经验。方法选取2020年3月1日开始于武汉大学人民医院妇产科接受2周线上实习+1周线下临床技能强化的2016级临床医学专业本科生156名,以及2021年3月1日开始于武汉大学人民医院妇产科轮流实习的2017级临床医学专业本科生共215名为研究对象,线上实习生采用抽签法随机分为A组(2周线上FC联合CBL实习教学+1周线下临床技能强化组)80名和B组(2周线上传统LBC实习教学+1周线下临床技能强化组)76名,线下实习生为C组(线下传统LBC实习教学组)215名。比较三组实习生出科考试成绩以及A、B两组线上实习疲劳度、对教学方式满意度及教学效果自评结果。结果在基础理论、临床案例分析以及技能操作部分A组成绩显著高于B组(P<0.05),而A、C两组无显著性差异;理论部分和总成绩均有A组>C组>B组,具有统计学意义(P<0.05)。线上实习生A组相比B组更不易出现疲劳,且对教学方式的满意度更高,对教学效果自我评价更高。结论线上传统教学模式对于妇产科实习生效果较差,无法达到预期。而采用线上FC联合CBL教学法可以明显提高实习生成绩,达到同线下实习相同的效果甚至更高的成绩。并且联合教学法有效减轻线上实习生的疲劳程度,在实习生中满意度更高,在后疫情时代具有重要的推广价值。
Objective To explore the effect of FC combined with CBL in clinical internship teaching of obstetrics and gynecology under the COVID-19 pandemic,and to summarize experience for improving the teaching efficiency and quality of clinical internship in obstetrics and gynecology in the post-pandemic era.Methods The experimental subjects are 156 undergraduates majoring in clinical medicine of grade 2016 who took an online internship in the Department of Obstetrics and Gynecology of Renmin Hospital of Wuhan University from March 1,2020 and 215 undergraduates majoring in clinical medicine of grade 2017 who took an internship from March 1,2021 offline.The online interns were randomly divided into two groups by lottery method.There are 80 interns in group A(online FC combined with CBL practice teaching for 2 weeks,and offline clinical skill strengthening training for 1 week),76 interns in group B(online traditional LBC practice teaching for 2 weeks,and offline clinical skill strengthening training for 1 week),and 215 offline interns in group C(offline traditional LBC practice teaching group)in order to compare the scores of the three groups after internship,as well as the results of the online practice fatigue,satisfaction with teaching methods,and self-assessment results of the two groups A and B.Results The scores of group A are significantly higher than those of group B in basic theory,clinical case analysis and operation(P<0.05),while there was no significant difference between groups A and C.With statistical significance(P<0.05),the scores of theoretical part and total scores of group A are higher than those of group C which are higher than those of group B.Compared with group B,online interns in group A are less prone to fatigue,are more satisfied with the teaching method,and have higher self-evaluation of the teaching effect.Conclusion The online traditional teaching mode is less effective for the obstetrics and gynecology interns and cannot meet expectations.The online FC method combined with CBL can significantly improve the performance of interns,and achieve the same effect as offline practice or even higher results.In addition,the joint teaching method can effectively reduce the fatigue of online interns,and improve the satisfaction of interns,which have important promotion value in the post-pandemic era.
