羊水过少孕产妇胎盘和胎膜中AQP9、HGF的表达变化及其与妊娠结局的相关性

Expression changes of AQP9 and HGF in placenta and fetal membrane of pregnant women with oligohydramnios and their correlation with pregnancy outcome

目的探讨羊水过少孕产妇胎盘和胎膜组织中水通道蛋白9(AQP9)、肝细胞生长因子(HGF)的表达变化及与妊娠结局的相关性。方法收集2017年3月至2021年3月在巴中市中心医院妇产科分娩的羊水过少孕产妇为观察组(n=114例),另收集同期羊水正常孕产妇为对照组(n=114例);采用免疫组化方法检测胎盘和胎膜组织中AQP9和HGF的表达,并收集两组孕产妇的妊娠结局,分析羊水过少孕产妇胎盘和胎膜中AQP9、HGF的表达水平与妊娠结局的相关性。结果两组孕产妇的胎盘和胎膜组织中均有AQP9和HGF的表达;观察组胎盘和胎膜组织中AQP9和HGF的表达水平均低于对照组(t值分别为27.03、33.31、18.17、12.27,P<0.01);观察组胎盘和胎膜组织中AQP9与HGF的表达均呈正相关(r=0.69,P<0.05;r=0.82,P<0.05);观察组不良妊娠结局总发生率(90.35%)明显高于对照组(37.72%),χ^(2)=68.56,P<0.01;观察组剖宫产率和产程异常率均高于对照组(χ^(2)值分别为35.86和10.28,P<0.01);羊水过少孕产妇胎盘和胎膜组织中AQP9和HGF的表达与剖宫产和产程异常的发生均呈负相关性(r值分别为-0.27、-0.34、-0.26、-0.36,P<0.05)。结论羊水过少孕产妇胎盘和胎膜组织中AQP9和HGF的表达水平较低,且AQP9与HGF的表达均呈正相关;羊水过少孕产妇剖宫产率、产程异常的发生率增加,而胎盘和胎膜组织中AQP9和HGF的表达与不良妊娠结局的发生亦呈负相关。

Objective To investigate the expression changes of aquaporin 9(AQP9)and hepatocyte growth factor(HGF)in placenta and fetal membrane of pregnant women with oligohydramnios and their correlation with pregnancy outcome.Methods From March 2017to March 2021,the pregnant women with oligohydramnios who delivered in the department of obstetrics,Bazhong Central Hospital were collected as the observation group(n=114),and the pregnant women with normal amniotic fluid during the same period were collected as the control group(n=114).The immunohistochemical methods were used to detect the expression of AQP9 and HGF in the placenta and fetal membrane,and the pregnancy outcomes of the two groups were collected.The expression levels of AQP9and HGF in the placenta and fetal membrane of pregnant women with oligohydramnios and their correlations with pregnancy outcomes were analyzed.Results AQP9and HGF were expressed in placenta and fetal membrane of pregnant women in both groups.The expression levels of AQP9and HGF in placenta and fetal membrane in the observation group were lower than those in the control group(t=27.03,33.31,18.17and 12.27,respectively,P<0.01).There was positive correlations between the expression of AQP9and HGF in placenta and fetal membrane of the observation group(r=0.69,P<0.05;r=0.82,P<0.05).The total incidence of adverse pregnancy outcomes in the observation group(90.35%)was higher than that in the control group(37.72%),χ^(2)=68.56,P<0.01.The cesarean section rate and abnormal labor process rate in the observation group were higher than those in the control group(χ^(2)=35.86and 10.28,respectively,P<0.01).The expression of AQP9and HGF in placenta and fetal membrane of pregnant women with oligohydramnios were negatively correlated with the occurrence of cesarean section and abnormal labor process(r=-0.27,-0.34,-0.26and-0.36,respectively,P<0.05).Conclusion The expression levels of AQP9and HGF in placenta and fetal membrane of pregnant women with oligohydramnios are lower,and the expression levels of AQP9and HGF are positively correlated.The incidences of cesarean section and abnormal labor process in pregnant women with oligohydramnios increase,and the expression of AQP9and HGF in placenta and fetal membrane are also negatively correlated with adverse pregnancy outcomes.
