悬吊训练联合生物反馈电刺激对产后盆底功能障碍早期康复的效果 被引量:7

Effect of suspension training combined with biofeedback electrical stimulation on early rehabilitation of postpartum pelvic floor dysfunction

摘要 目的探讨悬吊训练联合生物反馈电刺激对经阴道分娩产妇产后盆底功能障碍早期康复的效果。方法选取2019年8月至2021年8月南京医科大学附属淮安市第一医院收治的98例经阴道分娩产后盆底功能障碍产妇为研究对象,依照随机数字表法分为研究组和对照组,每组49例。对照组采用生物反馈电刺激治疗,研究组在对照组的基础上另给予悬吊训练。比较两组患者盆底肌功能影响评分、临床疗效、盆底神经肌电生理指标及压力性尿失禁发生情况。结果两组患者治疗后盆底肌功能影响评分均显著降低(t值分别为9.087、8.609,P<0.05),且研究组上述指标显著低于对照组(t=3.787,P<0.05);研究组患者治疗后总有效率显著高于对照组,差异有统计学意义(χ^(2)=4.780,P<0.05);两组患者治疗后快肌收缩最大值、10s慢肌收缩平均值均显著升高(t值分别为7.908、5.908、9.075、7.571,P<0.05),且研究组患者治疗后上述指标均显著高于对照组(t值分别为5.033、4.216,P<0.05);两组患者治疗后压力性尿失禁患病率均显著降低(χ^(2)值分别为47.680、32.337,P<0.05),且研究组患者治疗后显著低于对照组(χ^(2)=4.356,P<0.05)。结论悬吊训练联合生物反馈电刺激可显著改善盆底功能障碍产妇的盆底功能和盆底肌肌电指标,减少压力性尿失禁的发生。 Objective To investigate the effect of suspension training combined with biofeedback electrical stimulation on the early rehabilitation of postpartum pelvic floor dysfunction in vaginal delivery women.Methods A total of 98women with postpartum pelvic floor dysfunction,admitted to the Huai′an First People′s Hospital from August 2019to August 2021were selected as research objects.They were divided into study group(n=49)and control group(n=49)according to the random number table method.The control group was treated with biofeedback electrical stimulation,and the study group was additionally given suspension training on the basis of the control group.The pelvic floor muscle function impact score,clinical efficacy,electrophysiological indicators of pelvic floor nerve muscle and the occurrence of stress urinary incontinence were compared between the two groups.Results The pelvic floor muscle function impact scores in two groups were significantly decreased after treatment(t=9.087and 8.609,respectively,P<0.05),while the above indexes in study group were significantly lower than those in control group(t=3.787,P<0.05).After treatment,the total effective rate in study group was significantly higher than that in control group(χ^(2)=4.780,P<0.05).The maximum value of fast muscle contraction and average value of slow muscle contraction for 10sin the two groups were significantly increased after treatment(t=7.908,5.908,9.075and 7.571,respectively,P<0.05),while the above indexes in study group were significantly higher than the control group after treatment(t=5.033and 4.216,respectively,P<0.05).After treatment,the prevalence of stress incontinence in the two groups was significantly decreased(χ^(2)=47.680and 32.337,respectively,P<0.05),while that in the study group was significantly lower than the control group(χ^(2)=4.356,P<0.05).Conclusion Suspension training combined with biofeedback electrical stimulation for women with pelvic floor dysfunction after vaginal delivery can significantly improve pelvic floor function and the pelvic floor muscle electromyography,and reduce the occurrence of stress urinary incontinence.
