鼠神经生长因子联合生酮饮食治疗难治性癫痫患儿的效果

摘要 目的:观察鼠神经生长因子联合生酮饮食治疗难治性癫痫患儿的效果。方法:选取2019年1月至2020年6月该院收治的80例难治性癫痫患儿进行前瞻性研究,按照随机数字表法分为对照组和观察组各40例。两组均给予常规抗癫痫药物治疗,在此基础上,对照组给予生酮饮食治疗,观察组在对照组基础上联合鼠神经生长因子治疗。比较两组治疗效果,治疗前后血清学指标[神经元特异性烯醇化酶(NSE)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)]水平和适应行为指标[执行功能行为评定量表父母版(BRIEF-P)、儿童适应行为量表]评分,以及不良反应发生率。结果:观察组治疗总有效率为95.00%(38/40),高于对照组的75.00%(30/40),差异有统计学意义(P<0.05);治疗后,两组血清NSE、IL-6、hs-CRP水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组BRIEF-P评分均低于治疗前,且观察组低于对照组,两组儿童适应行为量表评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:鼠神经生长因子联合生酮饮食治疗难治性癫痫患儿可提高治疗总有效率,改善适应行为指标水平,降低血清学指标水平,效果优于单纯生酮饮食治疗。 Objective:To observe effects of mouse nerve growth factor combined with ketogenic diet on children with refractory epilepsy.Methods:A prospective study was conducted on 80 children with refractory epilepsy admitted to the hospital from January 2019 to June 2020.They were divided into control group and observation group according to the random number table method,40 cases in each.Both groups were given conventional antiepileptic drugs.On this basis,the control group was treated with ketogenic diet,while the observation group was treated with mouse nerve growth factor on the basis of that of the control group.The therapeutic effects,the serological index levels[neuron-specific enolase(NSE),interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP)],the adaptive behavior index[behavior rating inventory of executive function-parents(BRIEF-P),children’s adaptive behavior scale]scores,and the incidence of adverse reactions were compared between the two groups before and after the treatment.Results:The total effective rate of treatment in the observation group was 95.00%(38/40),which was higher than 75.00%(30/40)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of serum NSE,IL-6 and hs-CRP in the two groups were lower than those before the treatment,those of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).The BRIEF-P scores of the two groups were lower than those before the treatment,and that of the observation group was lower than that of the control group;the scores of children’s adaptive behavior scale in the two groups were higher than those before the treatment,and that of the observation group was higher than that of the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Mouse nerve growth factor combined with ketogenic diet in the treatment of the children with refractory epilepsy can improve the total effective rate of treatment,improve the levels of adaptive behavior indicators,and reduce the levels of serological indicators.Moreover,it is superior to single ketogenic diet therapy.
