操作性肌电生物反馈联合康复训练对卒中后上肢痉挛性偏瘫患者的效果

摘要 目的:探讨操作性肌电生物反馈联合康复训练治疗卒中后上肢痉挛性偏瘫患者的效果。方法:选取2020年1月至2021年10月该院收治的100例卒中后上肢痉挛性偏瘫患者进行前瞻性研究,按随机数字表法将患者分为对照组、观察组各50例。对照组患者应用常规康复训练,观察组在其基础上联合操作性肌电生物反馈治疗。比较两组治疗前后肌张力分级、上肢痉挛程度[临床痉挛指数(CSI)]、运动功能[Fugl-Meyer运动功能量表(FMA)]及生命质量[健康状况调查简表(SF-36)]。结果:治疗后,观察组肌张力分级、CSI评分均低于对照组,上肢FMA评分及SF-36各维度评分均明显高于对照组,差异有统计学意义(P<0.05)。结论:操作性肌电生物反馈联合康复训练用于卒中后上肢痉挛性偏瘫患者效果良好,可降低患肢肌张力和痉挛程度,改善患者肢体运动功能,提高其生命质量。 Objective:To explore effects of operantconditioning electromyography biofeedback combined with rehabilitation training on patients with upper limb spastic hemiplegia after stroke.Methods:A prospective study was conducted on 100 patients with upper limb spastic hemiplegia after stroke admitted to this hospital from January 2020 to October 2021.According to the random number table method,these patients were divided into control group and observation group,50 cases in each group.The control group was treated with routine rehabilitation training,while the observation group was treated with operantconditioning electromyography biofeedback on the basis of that of the control group.The muscle tone grade,spasm degree of upper limb[clinical spasm index(CSI)],motor function[Fugl-Meyer Motor Function Scale(FMA)]and quality of life(SF-36)were compared between the two groups before and after treatment.Results:After the treatment,the muscle tension grading and the CSI score of the observation group were lower than those of the control group;the upper limb FMA score and the SF-36 score were significantly higher than those of the control group;and the differences were statistically significant(P<0.05).Conclusions:Operantconditioning electromyography biofeedback combined with rehabilitation training is effective in the patients with upper limb spastic hemiplegia after stroke.It can reduce the muscle tension and spasm of the affected limb,improve the limb motor function and improve the quality of life of these patients.
