老年2型糖尿病患者三酰甘油血糖指数与早期动脉粥样硬化的相关性

Association of Triglyceride Glucose Index with Early Atherosclerosis in Older Patients with Type 2 Diabetes Mellitus

目的 探讨三酰甘油血糖(TyG)指数与老年2型糖尿病(T2DM)患者早期动脉粥样硬化的关系。方法 选取2018年1月~2020年10月收治的≥60岁的T2DM患者200例作为研究对象,根据颈动脉内膜中层厚度(CIMT)分为CIMT正常组(89例)和CIMT增厚组(111例),根据TyG指数三分位数将老年T2DM患者分为低TyG指数组(67例)、中TyG指数组(67例)和高TyG指数组(66例)。根据所测的老年T2DM患者的空腹血糖和三酰甘油,计算TyG指数,用CIMT增厚反映早期动脉粥样硬化,分析TyG指数和CIMT的关系。结果 CIMT增厚组TyG指数高于CIMT正常组(P<0.05);Pearson相关分析提示TyG指数与CIMT呈正相关(r=0.384,P<0.001);logistic回归分析提示TyG指数是CIMT增厚的独立危险因素(P<0.05);随着TyG指数的升高,CIMT逐渐升高(P<0.05),高TyG指数组CIMT值高于中TyG指数组和低TyG指数组(P<0.05),中TyG指数组和低TyG指数组间CIMT值比较,差异无统计学意义(P>0.05)。结论 TyG指数与老年T2DM患者早期动脉粥样硬化密切相关。

Objective To investigate the association of triglyceride glucose(TyG) index with early atherosclerosis in older patients with type 2 diabetes mellitus(T2 DM).Methods A total of 200 patients with T2 DM(aged≥60 years) treated from January 2018 to October 2020 were enrolled in this study.According to carotid intima-media thickness(CIMT),the patients were divided into a normal CIMT group(n=89) and a thickened CIMT group(n=111).According to TyG index, the patients were divided into a low TyG index group(n=67),medium TyG index group(n=67) and a high TyG index group(n=66).The TyG index was calculated according to fasting blood glucose and triglyceride values.Thickened CIMT was an indicator for early atherosclerosis.The association of TyG index with CIMT was analyzed.Results The TyG index was significantly higher in the thickened CIMT group than that in the normal CIMT group(P<0.05).Pearson correlation analysis showed that TyG index was positively correlated with CIMT(r=0.384,P<0.001).Logistic regression analysis showed that TyG index was an independent risk factor for thickened CIMT(P<0.05).The CIMT increased gradually with the increase of TyG index(P<0.05).The CIMT value in the high TyG index group was higher than that in the medium TyG index group and the low TyG index group(P<0.05).There was no significant difference between the medium TyG index group and the low TyG index group(P>0.05).Conclusion The TyG index is closely associated with early atherosclerosis in older patients with T2 DM.
