GDF-15及IL-1β基因多态性与糖尿病肾病患者并发导管感染的关联 被引量:3

Relationship between GDF-15 and IL-1β gene polymorphisms and catheter infection in patients with diabetic nephropathy

摘要 目的 分析生长分化因子-15(GDF-15)、白介素1β(IL-1β)基因多态性与糖尿病肾病(DN)患者并发导管感染的关系。方法 选取2018年12月-2020年12月内蒙古民族大学附属医院收治的136例DN患者为研究对象,根据有无并发导管感染分为DN合并导管感染组(n=36)和DN组(n=100),比较两组血清GDF-15、IL-1β水平,采用聚合酶链反应-限制片段长度多态(PCR-RFLP)方法对GDF-15、IL-1β基因rs1058587、-511位点进行多态性分析,采用Logistic回归分析评价GDF-15、IL-1β基因多态性位点与DN患者并发导管感染的关系。结果DN合并导管感染组血清GDF-15、IL-1β水平高于DN组(P<0.05);ROC曲线分析显示,血清GDF-15、IL-1β水平联合预测DN合并导管感染的敏感度为97.00%,AUC为0.935,均高于单独检测(P<0.05)。DN合并导管感染组GDF-15基因rs1058587 SNP GG基因型及G等位基因频率高于DN组,CC基因型及C等位基因频率低于DN组(P<0.05);GDF-15基因rs1058587 SNP G基因型(OR=1.958,95%CI:1.293~2.580)患DN合并导管感染的风险升高,而C基因型(OR=1.762, 95%CI:1.113~3.115)患DN合并导管感染的风险降低(P<0.05)。结论 GDF-15基因rs1058587 SNP位点G等位基因可能会增加DN患者并发导管感染的危险性;而IL-1β基因多态性与DN患者并发导管感染无关。 OBJECTIVE To analyze the relationship between gene polymorphisms of growth differentiation factor-15(GDF-15) and interleukin 1β(IL-1β) and catheter infection in patients with diabetic nephropathy(DN). METHODS A total of 136 patients with DN admitted to the Affiliated Hospital of Inner Mongolia University for Nationalities from Dec 2018 to Dec 2020 were enrolled as the research subjects. According to whether catheter infection occurred, the patients were divided into the DN complicated with catheter infection group(n=36) and DN group(n=100). Serum GDF-15 and IL-1β levels between the two groups were compared. The polymorphisms of rs1058587 and-511 in GDF-15 and IL-1 β genes were detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) method. The relationship between GDF-15 and IL-1 β gene polymorphisms and catheter infection in patients with DN was evaluated by multivariate Logistic regression analysis. RESULTS Serum GDF-15 and IL-1β levels in the DN complicated with catheter infection group were significantly higher than those in the DN group(P<0.05). ROC curve analysis showed that the sensitivity of serum GDF-15 combined with IL-1β to predict DN complicated with catheter infection was 97.00%, and the AUC was 0.935, both of which were significantly higher than those of single detection(P<0.05). The frequencies of GG genotype and G allele at rs1058587 in GDF-15 gene in the DN with catheter infection group were significantly higher than those in the DN group, and the frequencies of CC genotype and C allele were significantly lower than those in the DN group(P<0.05). The risk of catheter infection was significantly higher in patients carrying G genotype at rs1058587 in GDF-15 gene(OR=1.958, 95%CI: 1.293-2.580), and significantly lower in those carrying C genotype(OR=1.762, 95%CI: 1.113-3.115)(P<0.05). CONCLUSION allele at rs1058587 in GDF-15 gene may increase the risk of catheter infection in patients with DN, but there is no relationship between IL-1β gene polymorphism and catheter infection.
