Hp感染与胃黏膜Fas抗原和IFN-γ蛋白表达的关系及意义

Relationship between Hp infection and expression of Fas antigen and IFN-γprotein in gastric mucosa and its significance

目的探讨幽门螺杆菌(Hp)感染与胃黏膜Fas抗原、γ-干扰素(IFN-γ)蛋白表达的关系及意义。方法选取在解放军总医院诊疗的慢性浅表性胃炎患者43例、慢性萎缩性胃炎患者50例,肠上皮化生患者35例、不典型增生患者40例和胃癌患者45例,检测各患者胃黏膜Fas抗原、IFN-γ蛋白表达以及Hp感染情况。结果慢性浅表性胃炎、慢性萎缩性胃炎、肠上皮化生、不典型增生及胃癌组Hp感染率分别为67.44%、62.00%、62.86%、62.50%和68.89%,差异比较无统计学意义;慢性浅表性胃炎Hp阳性者Fas抗原为48.28%,高于Hp阴性者(P<0.05);慢性萎缩性胃炎Hp阳性者Fas抗原和IFN-γ表达阳性率分别为48.93%和45.16%,高于Hp阴性者(P<0.05);不典型增生Hp阳性者IFN-γ表达阳性率为80.00%,高于Hp阴性者(P<0.05);胃癌Hp阳性者IFN-γ表达阳性率为90.32%,高于Hp阴性者(P<0.05)。结论Hp感染可影响胃黏膜Fas抗原、IFN-γ蛋白表达上调,值得进一步研究。

OBJECTIVE To investigate the relationship between Helicobacter pylori(Hp)infection and the expression of Fas antigen andγ-interferon(IFN-γ)in gastric mucosa.METHODS 43 patients with chronic superficial gastritis,50 patients with chronic atrophic gastritis,35 patients with intestinal metaplasia,40 patients with atypical hyperplasia and 45 patients with gastric cancer in PLA General Hospital were selected,the expression of Fas antigen,IFN-γprotein and Hp infection in gastric mucosa were detected.RESULTS The Hp infection rates of chronic superficial gastritis,chronic atrophic gastritis,intestinal metaplasia,atypical hyperplasia and gastric cancer were 67.44%,62.00%,62.86%,62.50%and 68.89%,respectively,with no significant difference;The positive rates of Fas antigen of Hp positive patients with chronic superficial gastritis was 48.28%,significantly higher than that of Hp negative patients(P<0.05);The positive rates of Fas antigen and IFN-γin Hp positive chronic atrophic gastritis were 48.93%and 45.16%,which were significantly higher than those in Hp negative chronic atrophic gastritis(P<0.05);The positive rate of IFN-γin Hp positive dysplasia was 80.00%,which was significantly higher than that in Hp negative dysplasia(P<0.05);The positive rate of IFN-γexpression in Hp positive gastric cancer was 90.32%,which was significantly higher than that in Hp negative gastric cancer(P<0.05).CONCLUSION Hp infection can affect the expression of Fas antigen and IFN-γprotein in gastric mucosa,which is worthy of further study.
