短期生长抑素联合内镜下套扎术治疗食管静脉曲张出血的疗效及安全性

Efficacy and safety of short-term somatostatin combined endoscopic band ligation in treatment of esophageal varices bleeding

目的探讨短期生长抑素联合内镜下套扎术治疗食管静脉曲张出血的疗效及安全性。方法收集2018年1月至2022年2月于新疆医科大学第六附属医院住院进行生长抑素联合内镜下套扎术治疗的75例食管静脉曲张出血患者的临床资料,根据生长抑素使用的时间将患者分为短期组(n=41,持续使用生长抑素3 d)和长期组(n=34,持续使用生长抑素7 d)。比较两组患者的治疗效果和治疗期间药物相关不良反应发生情况。观察出院后1个月患者是否发生消化道再出血及治疗期间药物相关不良反应是否消失。结果出院时,两组患者的止血率均为100%。治疗结束时,两组患者的红细胞、血红蛋白浓度比较,差异均无统计学意义(P>0.05)。两组患者的止血时间、悬浮红细胞输血量比较,差异均无统计学意义(P>0.05)。短期组患者的住院时间短于长期组患者,差异有统计学意义(P<0.05)。结论采用短期生长抑素联合内镜下套扎术治疗食管静脉曲张出血时,持续使用3 d生长抑素的疗效与持续使用7 d生长抑素相当,而且能够缩短患者的住院时间。

Objective To explore the efficacy and safety of short-term somatostatin combined endoscopic band ligation in the treatment of esophageal varices bleeding.Method Clinical data of 75 patients with esophageal varices bleeding who were treated by somatostatin combined endoscopic band ligation in the Sixth Affiliated Hospital of Xinjiang Medical University from January 2018 to February 2022 were collected,and the patients were divided into short-term group(n=41,received 3 days of somatostatin)and long-term group(n=34,received 7 days of somatostatin)according to the duration of somatostatin use.The treatment effect and incidence of adverse drug reactions during treatment between the two groups were compared.Whether the patient has gastrointestinal rebleeding 1 month after discharge and whether the drug-related adverse reactions disappear during treatment were observed and compared.Result At discharge,the success rate of hemostasis in both groups was 100%.At the end of treatment,there was no significant difference for red blood cell and hemoglobin concentration in two groups(P>0.05).There were no significant differences for hemostasis time,transfusion volume of suspended red blood cell in two groups(P>0.05).The hospital stay of short term group was lower than that of long term group,and the difference was statistically significant(P<0.05).Conclusion When short-term somatostatin combined endoscopic band ligation is used to treat esophageal varices bleeding,the efficacy of 3-day somatostatin therapy is comparable to that of 7-day somatostatin therapy,and can shorten the hospital stay of patients.
