摘要
目的探讨颈动脉内膜-中层厚度(IMT)、斑块积分联合Tei指数在冠心病筛查中的应用价值。方法选取2019年5月至2020年9月营口市中心医院收治的114例疑似冠心病患者,均行颈动脉超声与心脏彩色多普勒超声检查,以冠状动脉造影为金标准,将其分为冠心病组与非冠心病组,按照病变支数的不同将冠心病组患者分为单支病变组、双支病变组与多支病变组。比较冠心病组与非冠心病组、不同病变支数组冠心病患者的[颈动脉内膜-中层厚度(IMT)、斑块积分及Tei指数]并进行相关性分析。采用受试者工作特征(ROC)曲线分析颈动脉IMT、斑块积分联合Tei指数在冠心病筛查中的诊断效能。结果冠心病组患者颈动脉IMT、斑块积分及Tei指数均明显高于非冠心病组患者(P<0.01)。多支病变组患者颈动脉IMT、斑块积分及Tei指数明显高于双支病变组与单支病变组患者,且双支病变组患者上述指标均明显高于单支病变组患者(P<0.01)。冠心病患者病变支数与颈动脉IMT、斑块积分、Tei指数均呈正相关(r=0.812、0.753、0.798,P<0.01)。颈动脉IMT、斑块积分、Tei指数单独及联合诊断曲线下面积(AUC)分别为0.781、0.765、0.703、0.897,联合诊断的AUC最大。结论颈动脉IMT、斑块积分联合Tei指数能够有效提高对冠心病的诊断效能,在冠心病筛查中具有重要的应用价值。
Objective To analyze the application value of carotid intima-media(IMT)thickness,plaque score and Tei index in the screening of coronary heart disease.Method A total of 114 patients with suspected coronary heart disease admitted in Yingkou Central Hospital were selected.All patients were examined by carotid ultrasound and cardiac color Doppler ultrasound,they were divided into coronary heart disease group and non-coronary heart disease group using coronary angiography as the gold standard.The patients in coronary heart disease group were divided into single-vessel lesion group,double-vessel lesion group and multi-vessel lesion group.The ultrasound examination indexes of carotid intima-media thickness(IMT),plaque score and Tei index in coronary heart disease group,non-coronary heart disease group and different lesions groups were compared,the correlation analysis was performed.The diagnostic efficacy of carotid IMT,plaque score and Tei index were analyzed by using the receiver operating characteristic(ROC)curve.Result Carotid IMT,plaque score and Tei index of carotid artery in coronary heart disease group were significantly higher than those in non-coronary heart disease group(P<0.01).The carotid IMT,plaque score and Tei index of carotid artery in the multi-vessel lesion group were significantly higher than those in the double-vessel lesion group and the single-vessel lesion group(P<0.01),and the above indicators in double-vessel lesion group were significantly higher than those of the single-vessel lesion group(P<0.05).The number of diseased vessels in patients with coronary heart disease were positively correlated with carotid IMT,plaque score and Tei index(r=0.812,0.753,0.798,P<0.01).The area under curve(AUC)of single and combined detection of carotid IMT,plaque score,Tei index was 0.781,0.765,0.703 and 0.897,respectively,with the highest AUC of combined detection.Conclusion Carotid IMT,plaque score combined with Tei index can effectively improve the diagnostic efficiency of coronary heart disease and have important application value in screening coronary heart disease.
