Study on DRG Cost of 564 Heart Failure Cases in Tertiary Grade A hospitals in 2020

目的:探究心力衰竭病例DRG费用的结构和影响因素预测。方法:回顾性选取昆明某三甲医院心内科2020年564例心力衰竭患者的住院病例DRG支付数据对住院费用的结构、影响因素和相关性进行分析。结果:心力衰竭的患者以老年男性居多,平均住院11.47天;45岁-60岁患者住院总费用均值最高,住院总费用平均值为19624.05元,中位数为10042.06元;住院费用较高相关因素有住院日和耗材费,耗材费占比较大(38.84%);89.72%的患者住院费用小于3万元区间;42例使用高值耗材,其均值为99108.46元,中位数为123292.00元。结论:DRG支付方式下,心力衰竭住院费用的影响因素有年龄、性别、住院日、耗材费、药费,耗材在该疾病治疗费中的占比较高。

Objective:To explore the structure and influencing factors of DRG cost inheart failure cases.Methods:The DRG pay⁃ment data of 564 cases with heart failure in the department of cardiology of a certain tertiarygrade A hospital in Kunming in 2020 were retrospectively selected to analyze the structure,influencing factors and correlation of hospitalization costs.Results:Most of the pa⁃tients with heart failure were elderly men,and the average hospital stay was 11.47 days.Patients aged 45-60 years had the highest mean total hospitalization cost,average on￥19,624.05 and median on￥10,042.06.The related factors of higher hospitalization cost were length of stay and consumables cost,consumables cost accounts for a large proportion(38.84%).89.72%of the patients’hospital⁃ization expenses are less than￥30,000.High value consumables were used in 42 cases which average on￥99,108.46 and median on￥123,292.00.Conclusion:Under the DRG payment method,the influencing factors of heart failure hospitalization cost include age,gender,length of stay,consumables cost and drug cost,and consumables account for a relatively high proportion of the treatment cost of this disease.
