首次确诊糖尿病老年患者衰弱现状及影响因素分析

Analysis of frailty status and influencing factors of elderly patients diagnosed with diabetes for the first time

目的了解首次确诊糖尿病的老年患者的衰弱现状,并探讨影响首次确诊糖尿病老年患者衰弱的影响因素。方法对2021年3月至2022年3月在该院内分泌科就诊的2型糖尿病患者135例进行横断面调查,采取一般资料表、衰弱量表、营养风险筛查量表、查尔森合并症指数等进行评价。结果首次确诊糖尿病的老年患者的衰弱总分为(2.83±0.75)分,衰弱发生率为36.30%,衰弱前期发生率为52.59%,无衰弱患者占比11.11%。Logistic回归分析显示,查尔森合并症指数、生活自理能力量表评分、文化程度、年龄、糖化血红蛋白值、营养风险得分是首次确诊糖尿病的老年患者衰弱的影响因素。结论首次确诊为糖尿病的老年患者的衰弱发生率较高,护理人员应该加强对首次确诊为糖尿病的老年患者衰弱的关注程度,制定个体化的衰弱干预措施,降低或延缓患者的衰弱发生程度。

Objective To investigate the frailty status of elderly patients diagnosed with diabetes for the first time,and to explore the influencing factors of frailty in elderly patients diagnosed with diabetes for the first time.Methods A cross-sectional survey of 135 patients with type 2 diabetes who were admitted to the Department of Endocrinology of this hospital from March 2021 to March 2022 was conducted.The general data sheet,fattening scale,nutritional risk screening Scale and Charleson comorbidities index were used to evaluate.Results The total score of frailty in the elderly patients diagnosed with diabetes for the first time was(2.83±0.75),the incidence of frailty was 36.30%,the incidence of pre-frailty was 52.59%,and the proportion of patients without frailty was 11.11%.Logistic regression analysis showed that Charlson Comorbidity Index,self-care ability scale score,educational level,age,hemoglobin a1c value and nutritional risk score were the influencing factors for frailty in the elderly patients with diabetes diagnosed for the first time.Conclusion The incidence of frailty in elderly patients diagnosed with diabetes for the first time is higher.The nursing staff should pay more attention to the frailty of elderly patients diagnosed with diabetes for the first time,and formulate individualized frailty intervention measures to reduce or delay the occurrence degree of frailty in patients.
