电动气压止血仪加压灌注在输尿管镜碎石术中的应用效果

Study on the application effect of pressurized perfusion with electric pneumatic hemostat in ureteroscopic lithotripsy

目的分析电动气压止血仪加压灌注在输尿管镜碎石术中的应用效果。方法选取2021年1月至2022年12月该院收治诊断为泌尿系结石患者800例作为研究对象,将其按照随机数字表法分为对照组和观察组,每组400例。手术巡回护士在输尿管镜钬激光碎石术中协助医生进行灌注冲洗,对照组予以医用加压器加压,观察组予以电动气压止血仪加压。对比分析围手术期指标、一期碎石成功率、结石排净率、卧床时间、住院时间及术后并发症发生率。结果与对照组术中视野清晰比较,观察组更高,差异有统计学意义(P<0.05);与对照组平均手术时间、术中冲洗量及肾盂压力增加度比较,观察组更优,差异有统计学意义(P<0.05);一期碎石成功率、结石排净率观察组较对照组更高,差异均有统计学意义(P<0.05);观察组卧床时间、住院时间及术后并发症发生率较对照组均更优,差异均有统计学意义(P<0.05)。结论在输尿管镜碎石术中加用电动气压止血仪加压灌注,可有效改善围手术期指标,提高一期碎石成功率及结石排净率,缩短卧床时间、住院时间,提高手术治疗安全性。

Objective To analyze the application effect of pressurized perfusion with electric pneumatic hemostat in ureteroscopic lithotripsy.Methods A total of 800 patients diagnosed with urinary calculi in this hospital from January 2021 to December 2022 were selected as research objects,and were divided into control group and observation group according to random number table method,with 400 cases in each group.Itinerant suigical nurses assisted doctors in perfusion and flushing during ureteroscopic holmium laser lithotripsy.The control group was pressurized by medical pressurizer and the observation group was pressurized by electric pneumatic hemostat.The perioperative indexes,the success rate of first-stage lithotripsy,the stone removal rate,time in bed,hospital stay,and the incidence of postoperative complications were compared and analyzed.Results Compared with the control group,the observation group had a higher intraoperative visual field,and the difference was statistically significant(P<0.05);the average operation time,intraoperative irrigation volume and the increase degree of renal pelvis pressure in the observation group were lower,and the differences were statistically significant(P<0.05);the success rate of first-stage lithotripsy and stone removal rate were higher,and the differences were statistically significant(P<0.05);the time in bed,the hospital stay,and the incidence of postoperative complications were lower,and the differences were statistically significant(P<0.05).Conclusion Adding pressurized perfusion with electric pneumatic hemostat in ureteroscopic lithotripsy can effectively improve the perioperative indexes,increase the success rate of first-stage lithotripsy and stone removal rate,shorten the time in bed,hospital stay and improve the safety of surgical treatment.
